3rd Shift: FT 40 hours - Monday - Friday 12 AM - 8 AM
Position: Clinician I
Program: REST
Location: New Haven
Scope of Service: Provides a comprehensive array of direct clinical services to adults and families with mental health, intellectual and/or addiction disorders. Counsels and provides psychotherapeutic and rehabilitation services in individual and group settings to facilitate independence with identified treatment goals toward symptom management. Carries a clinical case load as assigned, coordinates services, including assessments and triage of clients. Performs other duties as assigned.
ESSENTIAL JOB FUNCTIONS
Client/Clinical Care:
• Establishes rapport and positive working relationships with clients while
maintaining professional ethics and boundaries as a clinician.
• Carries clinical case load and provides clinical case management services,
including referrals to appropriate levels of care as assigned
• Assesses, counsels, and provides psychotherapeutic treatment, rehabilitation,
and care coordination for adults and families with mental illness and/or
addiction disorders.
• Conducts intake assessments/psychosocial, mental health status, and
diagnostic/functional assessments; translates this information into medically
necessary, multidisciplinary treatment plans.
• Develops and maintains individualized plans of care for each client assigned.
• Meets outpatient productivity standards as set forth by Continuum of Care, Inc.
Provides timely and accurate documentation for medical necessity billing of
services provided.
• Assures pre-authorizations are complete for clients and that medical necessity
standards are met.
• Facilitates/co-facilitates psycho-educational groups as indicated.
• Provides clinical triage to facilitate necessary services, such as inpatient
hospitalization, when indicated.
• Responds to crisis situations and provides appropriate interventions as outlined
in the agency policy for Nonviolent Practices.
• Collaborates with families, outside treaters, etc. to assure their integration into
the treatment planning process, as indicated.
• Is fully involved in quality assurance/program evaluation efforts, including
utilization of such data in day to day practice.
• Communicates actively with supervisor regarding any/all high-risk events, and
other concerns highlighted by Program Director.
• Completes other duties assigned.
Environmental Responsibilities:
• Maintains a clean, well-organized work environment including general office
maintenance and vehicle inspections.
• Ensures that all egresses and walkways are clear to support workplace safety.
• Participates in property up-keep and sanitation of medication areas, common
areas, kitchens, bathrooms, etc. as applicable to program site.
• Notifies supervisor of any maintenance and/or safety issues and generates
work orders as needed.
• Conducts and participates in fire and emergency evacuation drills, and reviews
the emergency preparedness plan as outlined in agency policy.
• Participates in Food Bank activities as assigned.
• Completes other designated environmental tasks as assigned.
Quality Assurance:
• Adheres to policies and practices as outlined in the staff handbook and
program operations manual.
• Consistently works in a respectful, constructive, professional, ethical, and
cooperative manner with fellow staff and providers.
• Upholds all Health Insurance Portability and Accountability Act (HIPAA)
regulations and secures all necessary releases of information for outside
communication.
• Models culturally competent behavior interacting with clients,
professionals/providers, and fellow staff.
• Maintains accurate electronic records and case files as assigned and in
accordance with programming, contractual, licensing, and accreditation
requirements.
• Adheres to the standards of the Council on the Accreditation of Rehabilitation
Facilities (CARF).
• Participates in agency committee work when available or as permitted.
• Completes all trainings as required per program contract and agency policy.
• Attends agency sponsored in-services, outside trainings, conferences and
meetings to facilitate continued professional growth and ensure the quality of
case management practices.
• Maintains license in good standing for which services delivered are required.
QUALIFICATIONS
• Master’s degree required with a minimum of one (1) year post graduate
experience working in the behavioral health field. Clinician I positions include
LMSW, LPCA, or MFT.
• Prior experience working with people with severe and persistent mental illness,
intellectual disabilities, and/or addiction.
• Valid Connecticut Driver’s license with clean record is required and must be
maintained.
• Knowledge of the community support system and resources is preferred.
• CPR/First Aid certification offered as part of orientation. Employee responsible
to maintain ongoing certification.
• Clinician required to maintain personal professional liability insurance.
• Proficient computer skills required, including Microsoft Office and electronic
record keeping.
