Summary of Job Description
The Mental Health Rehab Worker (MHRW) works with adults providing Adult Rehabilitative Mental Health Services (ARMHS). ARMHS is a program that provides intensive rehabilitative services to individuals ages 18+ who have a mental health diagnosis, in the form of instruction, support, and assistance for the purpose of promoting psychiatric stability, social competencies, personal and emotional adjustment, and independent living skills. The MHRW works directly under the ARMHS Treatment Director and/or ARMHS Clinical Supervisor.
Reach’s ARMHS program provides services to individuals living in Dakota, Hennepin, and Ramsey Counties. MHRWs meet with individuals in their homes or communities to build basic living and social skills. Basic living and social skills are activities that instruct, assist, and support the individual in skill areas essential for every day, independent living. This position will work at least 32 hours a week.
Essential Duties and Responsibilities
- Maintain client contact hours of a minimum of 26 hours per week.
- Goal to meet each client a minimum of once/week for two hours
- Adjust schedule to be responsive to client scheduling needs, including potential for late afternoon or evening appointments
- Provide services in the home or other community location or via interactive video
- Complete accurate case notes for each visit concurrently in the meeting when possible. Must be completed on date of service.
- Ensure confidentiality of all client records and interactions
- Attend all required team supervision and company meetings and participate in professional meetings and workshops
- Participate in team-based decisions regarding all clients and services
- Maintain current knowledge of the programs and services Reach provides
- Public Relations/Networking/Liaison
- Responsible for the reputation and public image of the agency
- Develop and maintain positive working relationships within the community
- Refer clients to other professionals as needed with supervisor approval
- Communicate with other providers involved
- Participate in other Reach for Resources functions as assigned
Working Conditions and Physical Demands
· Environment: Office environment, remote work, or in-field meetings.
· Schedule: Weekdays, may be flexible depending on individuals served and employee schedule.
· Physical demands: Light lifting (under 20lbs), sitting and standing, walking, driving, verbal communication, seeing, and hearing.

Requirements:Minimum Qualifications
Candidates must meet state criteria for classification as a Mental Health Rehab Worker Minnesota Statutes 245I.04.
Mental Health Rehabilitation workers must have a high school diploma or equivalent and meet one of the following:
- Be fluent in the non-English language or competent in the culture of the ethnic group to which at least 20 percent of the mental health rehabilitation worker's clients belong, or
- Have an associate of arts degree, or
- Have two years of full-time postsecondary education or a total of 15 semester hours or 23 quarter hours in behavioral sciences or related fields, or
- Be a registered nurse, or
- Have, within the previous 10 years, three years of personal life experience with mental illness, or
- Have, within the previous 10 years, three years of life experience as a primary caregiver to an adult with a mental illness, traumatic brain injury, substance use disorder, or developmental disability, or
- Have, within the previous 10 years, 2,000 hours of work experience providing health and human services to individuals
- Must have the ability to work independently, enjoy flexibility, and manage stress.

- Must be able to act in a manner that encourages change and provides a positive and supportive atmosphere while identifying client needs and treatment goals.
- Practice effective decision making
- Handle crisis situations and follow through of supervisors’ directives
- Work with culturally diverse populations
- Comfortable learning and using Procentive, Microsoft 365, and other computer programs used by the company
- Must be able to understand and communicate the English language
- Must be able to use and maintain a daily schedule
- Must have current driver’s license in good standing and maintain own reliable transportation.
