Job Title – Social Enterprise and Outreach Manager

We are seeking a detail-oriented and motivated Social Enterprise and Outreach Manager to advance HHM Health's mission through community outreach, community engagement, population health initiatives, behavioral health support, counseling services, health equity programs, and community partnership development. This position serves as HHM Health's primary community-facing leader and is responsible for building meaningful relationships throughout the communities HHM Health serves. Social Enterprise and Outreach Manager develops and implements innovative outreach strategies that connect individuals and families to healthcare services, preventive screenings, behavioral health resources, social services, and community supports while helping improve access to care and reduce healthcare disparities

Location: Dallas (5750 Pineland Dr. Texas 75231)

Here's a sneak peek at what you'll do:

  • Direct HHM Health's outreach strategy, community events, health fairs, screenings, wellness initiatives, and engagement campaigns to increase awareness and access to services.
  • Build andmaintainstrategic relationships with schools, universities, nonprofits, faith-based organizations, public agencies, employers, and community stakeholders.
  • Deliver psychosocial assessments, counseling, crisis intervention, and behavioral health support while promoting trauma-informed and resilience-based care.
  • Assistpatients in accessing healthcare and social services, coordinate referrals, and address barriers such as transportation, housing, health literacy, and continuity of care.
  • Develop school-based partnerships and provide educational workshops focused on wellness, preventive care, behavioral health, chronic disease management, and social determinants of health.
  • Lead initiatives that reduce healthcare disparities, improve access for underserved populations, and support quality improvement, HEDIS, and population healthobjectives.
  • Analyze community needs, develop innovative programs, support grant opportunities, and recommend service expansion and community impact strategies.
  • Track outreach outcomes,maintaincommunity impact dashboards, present findings to leadership, incorporate community feedback into planning, andutilizemobile health services to expand community access.

What you need to succeed

To be a productive member of our team, you will have a pleasant and professional demeanor, be a self-starter, the ability to work independently, strong communication skills, and the ability to preserve confidentiality. You will also have the following:

  • Master's degree in Social Work(MSW) from an accredited institution and active Texas LMSW or LCSW licensurerequired.
  • Minimum five years of experience in social work, behavioral health, counseling, community engagement, public health, care coordination, population health, or a related field.
  • Demonstrated success developing community partnerships and serving underserved populations.
  • Strong counseling, communication, presentation, facilitation, training, and relationship-building skills.
  • Experience collaborating with interdisciplinary healthcare and behavioral health teams.
  • Strong organizational, project management, and problem-solving abilities, withproficiencyin Microsoft Office and electronic health record systems.
  • LMSW candidates are eligible and will receive clinical supervision, with an expectation to obtain LCSW licensure within six months of employment.
  • Preferred experience includes FQHCs, community health centers, public health, schools, nonprofits, health equity initiatives, HEDIS/UDS reporting, grant writing, program development, trauma-informed care, and working with children, adolescents, and families.
