About the Role:
The LTFC Case Manager oversees the minor's case, service delivery, concurrent planning and reunification process. The LTFC Case Manager acts as the liaison between the program and all stakeholders involved in the child’s case. This role also ensures holistic support with the child’s treatment team as the minor continues in the existing placement.
What You’ll Do
- Conduct comprehensive case management services in compliance with all Federal Office of Refugee Resettlement (ORR), New York State Office of Children and Family Services (OCFS), and program requirements from admission through discharge.
- Complete intake assessments, orientations, foster home visits, safety and well-being assessments, and ongoing case planning to support each youth's successful placement and progress.
- Coordinate medical, educational, behavioral health, legal, and community-based services while maintaining regular communication with service providers to ensure continuity of care.
- Facilitate family reunification by engaging sponsors, collaborating with the Division of Sponsor Administration (DSA), and managing all required documentation and approval processes.
- Monitor immigration court requirements and collaborate with legal service providers to support timely submission of documentation related to immigration relief.
- Maintain accurate, timely, and confidential case documentation in MyEvolv (EHR), the UC Portal, and other required systems, ensuring compliance with ORR and OCFS standards.
- Prepare, redact, organize, and submit case records, discharge documentation, record requests, CA/N background check requests, and other administrative reports as required.
- Collaborate closely with foster families, clinical, medical, educational, and multidisciplinary teams to ensure coordinated, high-quality care for every youth.
- Transport youth, when needed, to medical appointments, court appearances, educational programs, family reunification meetings, and other program-related activities.
- Participate in an on-call schedule that may include evening and weekend hours while maintaining the highest standards of professionalism, compliance, and youth-centered service delivery.
Other related duties as assigned by supervisor.

Required Qualifications
- Bachelor’s in a Human Services field.
- Prior Case Management experience required.
- Knowledge of child welfare and immigration related legislation
- Excellent verbal, written communication and organizational skills are essential
- Proficient in Microsoft Office suite, knowledge of Connections
- Bilingual (English/Spanish )
Position Type/Expected Hours of Work
This is a full- time position. Days and hours of work are Monday through Friday at 317 Lenox Ave New York, NY office, in Manhattan. Work hours options are 9:00 am - 5:00 pm.
