Patient Care Coordinator (PCC)Create Exceptional Patient Experiences. Drive Clinic Success. Be the Front Door to Better Care.

The Patient Care Coordinator is more than the person at the front desk.

You are the first and last impression patients have of Peak Performance Physical Therapy. You set the tone, build trust, keep the clinic running efficiently, and help patients take the next step toward achieving their health goals.

This role is ideal for someone who loves connecting with people, solving problems, and making a measurable impact every day. You’ll combine customer service, organization, communication, and business awareness to create an exceptional experience for patients while helping the clinic achieve its goals.

What Success Looks LikeCreate an Exceptional Patient Experience
  • Welcome patients and create a positive, professional, and memorable experience from the first interaction.
  • Build relationships with patients by listening, answering questions, and providing guidance throughout their care journey.
  • Educate patients on available services, programs, and treatment options that support their goals.
  • Address concerns quickly and professionally to maintain patient trust and satisfaction.
Drive Clinic Performance
  • Manage scheduling to maximize provider productivity and ensure patients receive timely care.
  • Reduce cancellations and no-shows through proactive appointment management.
  • Help convert referrals into scheduled evaluations by creating a seamless patient experience.
  • Support clinic growth by identifying opportunities to better serve patients through additional services and programs.
Own Administrative Excellence
  • Maintain accurate patient, insurance, billing, and demographic information.
  • Collect payments accurately and professionally.
  • Support daily clinic operations through organization, communication, and follow-through.
  • Maintain compliance with HIPAA, payer requirements, and company policies.
Represent Peak Performance in the Community
  • Educate patients, referral partners, and community members about our services.
  • Communicate the value of physical therapy and available programs.
  • Serve as an ambassador for the Peak Performance brand.
