Primary Health Care (PHC) was founded in 1981 by Dr. Bery Engebretsen in Des Moines, IA. Our mission has remained unchanged since that time, to provide healthcare and supportive services to all, regardless of insurance, immigration status, or ability to pay.

Based on the needs of the communities we serve, PHC offers a spectrum of medical and dental services including family practice, behavioral health, HIV care and services, and pharmacy. PHC’s Homeless Support Services is the entry point for serving people experiencing homelessness in Polk County. Enabling services are available to help patients with benefits enrollment, case management, transportation, translation, and patient education. We currently have locations in Ames, Des Moines, & Marshalltown.




As a Housing Case Manager, you will be responsible for assisting The Project of Primary Health Care clients with housing needs. Develops and maintains relationships with landlords to secure housing units for people needing a place of residence. Provides ongoing support by serving as the liaison between landlords and clients. Assists in management of financial assistance received from Housing Opportunities for People Living with AIDS (HOPWA) and Ryan White housing supportive services. Assists in grant activities associated with housing support grants, such as annual RFA/RFP’s, CAPER and grant narratives that demonstrate continued need for housing services. Identifies opportunities to build capacity for affordable housing options.

What You Will Do

  1. Responsible for assisting clients with finding and maintaining appropriate housing options.
  2. Develops and maintains open communication with community partners, landlords, neighborhood groups, and other organizations to build capacity for affordable housing options, coordinate services and provide education about PHC services.
  3. Manages a housing assistance budget according to grant guidelines.
  4. Monitors and documents client contacts, housing goals, progress, and significant occurrences, according to grant guidelines and PHC policies and procedures.
  5. Collects, records, and reports data related to housing clients.
  6. Facilitates coordination with other PHC services, including medical, dental, mental health and substance abuse, as well as referrals for services with external agencies, including emergency and crisis services.
  7. Educates PHC staff about program focus, landlord/tenant issues, housing barriers, and related services.
  8. Performs other duties as assigned.


Qualifications You Need to Bring

Required:

  • Bachelor’s degree in human services field or equivalent combination of education experience.
  • Minimum of 2 years of experience working with clients who need assistance relating to homelessness, housing, and/or advocacy for underserved populations.
  • Excellent customer service skills.
  • Effective written and verbal communication skills.
  • Proficiency using Microsoft Office applications including Excel and other database applications.
  • Problem solving and critical thinking skills.
  • Ability to multi-task.
  • Demonstrates professionalism with ability to maintain composure in all situations and show empathy.
  • Licenses & Certifications: Current Mandatory Reporter Training: Child and Dependent Adult Abuse recognized by the Iowa Department of Public Health; Active Chauffer’s license from Iowa Department of Transportation. Both must be obtained within introductory period if not current.

Preferred:

  • Master’s degree in human services field.
  • Community Health Center experience.
  • Experience in housing related occupation.
  • Continuing education in housing and homelessness.
  • Bilingual in English and Spanish.
  • Knowledge of rental assistance and housing programs.
  • Knowledge of Ryan White and HIV/AIDS.
  • Working knowledge of local community resources and housing resources.

Your experience and skills determine your base pay. The hiring range for this position is typically $18.65 - $23.32 per hour. Candidates with extensive work experience related to this position may be considered for additional compensation up to the pay grade maximum of $27.98 per hour. PHC also offers a comprehensive benefits package, including:
