Job Description

SUMMARY:

The Shelter Case Manager plays a key role in supporting residents by assessing needs, developing individualized service plans, and connecting clients to housing, medical, mental health, and employment resources. This position focuses on empowering individuals toward stability and self-sufficiency while maintaining accurate documentation, ensuring compliance with program requirements, and collaborating with a multidisciplinary team to provide comprehensive, client-centered care.


REPORTING RELATIONSHIP:

Shelter Case Manager reports to the Director of Social Services


SCHEDULE:

10AM - 6PM

11AM - 7PM

12PM - 8PM

1-2 Late days per week , one weekend day (selected sites)


LOCATIONS:

Par Central - Jamaica

Wyndham - Long Island City


POSITION TYPE:

Fulltime Positions, one weekend day and 1-2 late days per week, selected sites may require a weekend day.


RESPONSIBILITIES IN THIS ROLE:

    Develop and monitor individualized service and treatment plans, including regular resident check-ins, assessments, and crisis intervention as needed

    Connect residents to community resources, social supports, and essential services that promote independence and well-being

    Coordinate and track services, including follow-ups with referrals and ongoing communication with providers

    Maintain accurate documentation, resident records, and provide timely reports to supervisors

QUALIFICATIONS:

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.     BS/BA Degree in a human services related discipline and 3-5 years’ experience working with applicable populations.

    Experience working with unhoused populations

    Demonstrates strong interpersonal, communication, and problem-solving skills with sound judgment, discretion, and the ability to manage multiple priorities and deadlines

    Maintains a professional, courteous, and customer service–oriented approach with a positive attitude and attention to detail

    Remains calm under pressure, able to identify and resolve routine issues effectively in fast-paced or high-stress environments

    Works independently and collaboratively, fostering a professional and supportive environment for staff, clients, and stakeholders
