Job Description
Mental Health Kokua (MHK) is hiring a Part-Time Licensed Practical Nurse (LPN) to support our Iwilei Kauhale program in Honolulu, Oahu. This on-site role provides practical nursing support, medication coordination, and recovery-oriented care for adults experiencing mental health and housing challenges within a supportive housing environment.
As an LPN, you will help maintain medication systems, coordinate with pharmacies and providers, support continuity of care, and work closely with interdisciplinary staff to promote wellness and stability. This role is ideal for nurses who value relationship-based care, strong teamwork, and meaningful community impact in a behavioral health setting.
What You’ll Do (Responsibilities)
- Medication Coordination: Maintain and update Medication Administration Records (MARs) and support medication reconciliation
- Pharmacy & Prescriber Communication: Coordinate refills, medication follow-up, and communication with pharmacies and providers
- Client Support: Promote medication adherence, wellness routines, and continuity of care
- Health Monitoring: Observe and report medication response, side effects, and changes in client condition
- Documentation: Complete nursing notes, medication logs, incident reports, and required documentation accurately and timely
- Care Collaboration: Work closely with Housing Specialists, case managers, hospitals, clinics, and treatment providers
- Medication Safety: Support medication storage, labeling, tracking, and documentation procedures
- Team Participation: Participate in treatment team meetings, trainings, and quality improvement efforts
Licensed Practical Nurse Qualifications
- Minimum: Current Licensed Practical Nurse (LPN) license in the State of Hawaiʻi and one (1) year of nursing experience
- Preferred: Experience in behavioral health, residential services, community health, medication coordination, or human services settings
- Skills: Strong communication, organization, documentation, problem-solving, and teamwork skills
- Certifications: Current CPR, First Aid, and TB clearance (or ability to maintain current status)
- Fit for the Role: Comfortable working in a supportive housing environment serving adults experiencing mental health and housing challenges
Requirements & Work Conditions
- Part-time schedule anticipated to be fewer than 20 hours per week
- Ability to pass a criminal background check and drug/alcohol screening
- Current Hawaii driver’s license, vehicle insurance, and safety inspection may be required based on program needs
- Ability to lift up to 25 lbs and work in a residential community-based setting
- Work involves standing, walking, documentation, medication coordination, and communication with community providers
