About the Role

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our interdisciplinary clinical team. Working under the direction of a Registered Nurse, the LPN plays a vital role in delivering evidence-based preventative, therapeutic, and rehabilitative nursing care to assigned clients. In this role, you will provide direct hands-on nursing, facilitate health and wellness training for clients and their families, and collaborate closely with team members to promote physical, psychosocial, and spiritual well-being.

In this role, you will perform direct hands-on care, participate in clinical assessments, educate clients and their families, and train care team members on health strategies designed to promote physical, psychosocial, and spiritual well-being.

Essential Duties & Responsibilities

Direct Client Care & Clinical Support

  • Provide safe, high-quality preventative, therapeutic, and rehabilitative nursing care under established policies, guidelines, and RN oversight.
  • Perform physical and psychosocial nursing assessments, identifying clients in need of medical services and executing timely interventions.
  • Support clients in complex or difficult situations, including medication management, treatment refusals, and behavioral strategies.
  • Offer assistance ranging from minimal supervision to direct care to help clients achieve their treatment goals.
  • Support direct care needs at program sites or accompany clients to medical appointments as needed.
  • Care Coordination & Interdisciplinary Collaboration
  • Participate actively in interdisciplinary team decision-making, treatment planning, and goal setting.
  • Communicate essential medical and nursing information to physicians, healthcare providers, and team members.
  • Ensure consistent execution of care strategies by reviewing medical records, trackers, and lab results, and gathering feedback from staff.

Family & Staff Education

  • Train and educate clients, families, employers, and care constituents on health promotion, wellness, and medical stability.
  • Assist in developing individualized teaching and discharge plans to smooth transitions and restore function.
  • Lead health-related orientations and training sessions for staff members to support client care plans.

Documentation & Compliance

  • Complete all medical documentation promptly, including nursing progress notes, medication logs, and clinical assessments.
  • Accurately log the date, time, and length of all client interactions within the company system of record.
  • Maintain strict adherence to state licensing regulations, clinical standards, and professional codes of conduct.
  • Engage in continuing education to remain current with specialized treatment modalities and industry standards.

Qualifications & Requirements

  • Licensure: Active State Licensure as a Licensed Practical Nurse (LPN) or equivalent required.
  • Experience: Experience in the rehabilitation of chronically or catastrophically ill or injured individuals preferred.
  • Skills: Strong clinical judgment, problem-solving abilities, and time management skills.
  • Communication: Excellent verbal and written communication skills for interacting with clients, families, clinical teams, and physicians.
  • Operational Requirements: Valid driver’s license with a record of responsible driving behavior and ability to lift up to 50 lbs.
