Job Description

Description:

This includes but is not limited to:

  • Perform focused data collection and nursing observations, contributing to patient assessments with RN review and provide ongoing health monitoring while promptly reporting changes in condition to support patient stability within ASAM 3.7/3.5
  • Identify and promptly report changes in medical or psychiatric status, including early indicators that may require escalation
  • Support safe management of co-occurring conditions through observation, communication, and collaboration with the medical team
  • Administer medications in accordance with physician orders, ensuring adherence to the “5 rights” of medication administration
  • Monitor for medication effectiveness and side effects, documenting and communicating findings to the RN/Medical Provider
  • Provide patient education on medications, promoting understanding, compliance, and engagement in recovery
  • Complete timely, accurate, and clinically relevant documentation in the EMR(Sunwave), reflecting patient status, interventions, and response to care
  • Ensure documentation supports medical necessity, program fidelity, and continuity of care, reducing risk of denials and unnecessary discharges
  • Maintain compliance with DPH state regulations, accreditation standards (e.g., Joint Commission), and Emend-CT policies
  • Actively engage with patients on the milieu, reinforcing treatment goals, recovery principles, and use of coping strategies
  • Collaborate with therapists and behavioral health staff to support interventions prior to discharge, particularly for patients at risk of AMA/ATA
  • Contribute to a safe, structured, and trauma-informed environment that promotes retention and positive treatment outcomes
  • Communicate effectively with nursing, clinical, admissions, and leadership teams regarding patient status and needs
  • Participate in shift report, clinical rounds, and treatment team meetings as appropriate
  • Support continuity of care through clear handoff communication and coordination with Utilization Management when applicable
  • Recognize and respond to patient presentations involving cravings, behavioral escalation, or psychiatric distress using appropriate interventions
  • Escalate concerns in real time, following established workflows, rather than relying solely on passive notification processes
  • Support implementation of safety plans, including observation levels when indicated
  • Adhere to Emend-CT clinical programming structure and daily schedule, supporting consistency in care delivery
  • Reinforce expectations around patient participation, structure, and accountability within the residential setting
  • Contribute to quality improvement efforts, including participation in audits, feedback loops, and Quality Assurance Performance Improvement (QAPI)-related initiatives
  • Other Duties as Assigned by RN, Supervisor or Emend-CT administration.
Requirements:
  • Current LPN license in the state of Connecticut (in good standing)
  • Current CPR and First Aid Certification
  • Current Naloxone Certification (or ability to obtain upon hire)
  • Current TB clearance in accordance with state regulations
  • Minimum one (1) year of licensed nursing experience preferred
  • Experience in a residential treatment setting and/or substance use disorder (SUD) treatment strongly preferred
  • Demonstrated experience working with co-occurring mental health and substance use disorders
  • Experience with medication administration and monitoring in a behavioral health setting
  • Familiarity with withdrawal symptom identification and management (within scope of ASAM 3.7/3.5)
  • Experience with electronic medical records (EMR) documentation
  • Working knowledge of chemical dependency, addiction, and recovery principles
  • Ability to identify changes in patient condition and escalate appropriately
  • Strong understanding of HIPAA, patient rights, and confidentiality standards
  • Demonstrates clinical judgment, attention to detail, and adherence to program fidelity
  • Excellent verbal and written communication skills
  • Strong time management skills with the ability to prioritize patient needs in a fast-paced, 24-hour environment
  • Ability to work independently while collaborating effectively within an interdisciplinary team
  • Demonstrated problem-solving ability and critical thinking skills
  • Adaptable and flexible in a dynamic treatment environment
  • High level of integrity, professionalism, and patience
  • Proficient in Microsoft Office and Electronic Medical Record Documentation
  • Accurate and timely clinical documentation skills
  • Ability to move throughout the facility to observe and support patients and staff
  • Adequate vision, hearing, manual dexterity, and coordination to safely perform nursing duties
