Description:

The Floating Hospital, a non-profit healthcare organization located in Long Island City (Queens), is seeking a Case Manager to join its Behavioral Health team on a full-time basis. The Case Manager will be responsible for maintaining a patient caseload of BH patients, including but not limited to mental health concerns, substance abuse concerns, the need for life skills intervention, and comprehensive health concerns.

ESSENTIAL FUNCTIONS:

Patient Care:

· Conduct patient outreach following referrals to assess behavioral health needs, evaluate symptoms and risk factors, determine program eligibility, and schedule Behavioral Health Intake and Psychiatric Initial Assessment appointments.

· Coordinate same-day referrals and collaborate with primary care providers to ensure timely patient engagement and continuity of care.

· Participate in weekly multidisciplinary team meetings to discuss patient care, strengthen interdepartmental communication, improve patient flow, and promote continuity of care across departments

· Maintained accurate referral and emergency transfer logs while ensuring timely, thorough, and compliant documentation in accordance with organizational and regulatory standards.

· Complete comprehensive and timely case management documentation in the electronic health record (EHR) in compliance with organizational and regulatory standards.

· Support clinical staff during behavioral health emergencies by assisting with crisis response protocols and patient care coordination.

· Coordinate referrals and appointments for patients requiring higher levels of care or specialized community-based services.

· Conduct monthly provider roster reviews to maintain accurate provider information and support operational efficiency.

· Perform patient outreach for missed appointments, rescheduling visits whenever possible and mailing follow-up letters when phone contact is unsuccessful.

· Collaborate with case managers and multidisciplinary professionals from healthcare providers, hospitals, schools, child welfare agencies, foster care organizations, and community-based organizations to coordinate patient care, facilitate communication, and ensure continuity of services.

· Provide scheduling support in the absence of the Behavioral Health Scheduler by confirming appointments, verifying insurance eligibility, primary care provider information, copayments, outstanding balances, and updating patient demographic information.

· Process pharmacy requests by coordinating prescription refills and medication orders as authorized by psychiatric providers.

· Perform additional administrative, care coordination, and patient support responsibilities as assigned.

Key Qualifications

· Demonstrated empathy and cultural sensitivity when serving low-income, homeless, elderly, and underserved individuals and families.

· Extensive knowledge of New York State social service programs, including HRA, SNAP, Public Assistance, WeCARE, Fedcap, housing voucher programs, and other community resources.

· Skilled in care coordination, patient advocacy, crisis intervention, and interdisciplinary collaboration.

· Proficient in Electronic Health Records (EHR), medical documentation, scheduling systems, and behavioral health workflows.

· Bilingual in English and Spanish (preferred).

Requirements:

Background and Education Requirement:

Bachelor’s Degree preferred with two years of relevant experience; direct service experience, especially in crisis intervention, case management, emergency / traditional housing, and substance abuse. Must be used to working in a high-volume environment, must be courteous to all levels of staff and management, and must possess superior telephone skills.
