Registered Nurse (RN) - Erie County - Part Time with Benefits in Niagara Falls, New York at The HomeCare Partners

Duties & Responsibilities:

• Provide assessment of newly referred patients and assess health needs in home care setting
• Work with patient to create a patient centered plan of care
• Reassess patient needs for services on a continual basis and discuss any changes in the plan of treatment with the patient physician/family
• Prepare written documentation and summaries of the patient’s progress as needed for the continuation of home health services
• Provide supervision and training of PCA/HHA staff

Qualifications:

• NYS Licensed Registered Professional Nurse
• Minimum of Associate's Degree required
• One year Nursing experience required
• Ability to travel throughout Erie County for home based visits
• Home care experience a plus
