About the Role:

The Community Support Provider plays a vital role in enhancing the quality of life for individuals within the community by offering personalized assistance and support services. This position focuses on fostering independence, promoting social inclusion, and ensuring access to necessary resources for community members. The role requires building trusting relationships with clients, understanding their unique needs, and collaborating with multidisciplinary teams to develop and implement effective support plans. The Community Support Provider acts as an advocate, helping individuals navigate social services, healthcare, and other community resources. Ultimately, this position aims to empower individuals to achieve their personal goals and improve their overall well-being within the community setting.

Minimum Qualifications:

  • High school diploma or equivalent.
  • Experience working in a community support, social services, or related role.
  • Valid driver’s license and reliable transportation.
  • Ability to pass background checks and comply with state and federal regulations.
  • Strong communication and interpersonal skills.

Preferred Qualifications:

  • Certification in First Aid and CPR.
  • Experience working with diverse populations, including individuals with disabilities or mental health challenges.
  • Knowledge of local community resources and social service systems.
  • Bilingual abilities or proficiency in a second language.

Responsibilities:

  • Provide direct support and assistance to individuals in their homes and community settings to promote independence and well-being.
  • Develop and implement individualized support plans in collaboration with clients, families, and healthcare professionals.
  • Assist clients with daily living activities, including but not limited to personal care, transportation, and accessing community resources.
  • Monitor client progress and document services provided, ensuring compliance with organizational policies and regulatory requirements.
  • Advocate for clients’ needs and rights within the community and coordinate with external agencies to facilitate access to services.

Skills:

The required skills such as strong communication and interpersonal abilities are essential for building rapport with clients and collaborating effectively with team members and external agencies. Organizational skills are used daily to manage client documentation, schedule appointments, and coordinate services efficiently. Problem-solving skills help in addressing client challenges and adapting support plans to meet evolving needs. Preferred skills like knowledge of community resources and bilingual abilities enhance the provider’s capacity to connect clients with appropriate services and communicate effectively across diverse populations. Additionally, certifications in First Aid and CPR ensure the provider can respond promptly and safely to emergencies, contributing to client safety and well-being.

GENERAL DESCRIPTION OF DUTIES AND RESPONSIBILITIES:
Provides services to individuals with Intellectual and Developmental Disabilities living in their homes. May require handling individuals that are non-ambulatory or that require total assistance with hygiene, bathing, housekeeping, lifting, and transferring, and other services as needed. May provide transportation to medical appointments, outings, and other community access activities. Includes working with individuals with high intensity needs. This transportation will include travel out of the area and to other cities as required.

MINIMUM JOB QUALIFICATIONS:

Applicants must meet the following minimum qualifications to be eligible for the position:

High school diploma, GED, or the ability to pass a Comprehension Test and provide 3 non-family letters of reference.
Clear driving record and current TDL.
Must be able to travel either by agency vehicle or personal vehicle to transport individuals out of town.
Must be available to work a flexible schedule, including weekends, based on the agency’s needs.
Able to speak and write both English and Spanish.

PREFERRED JOB QUALIFICATIONS:

Experience working with people with Intellectual and Developmental Disabilities.

KNOWLEDGE, SKILLS, AND ABILITIES:

Ability to communicate effectively.
Ability to write clearly and document all contacts.
Ability to work effectively with individuals with special needs.
Ability to work under conditions requiring physical stamina and ability to work in a stressful environment.
Must possess good computer skills.
