Position: Intellectual Disabilities Supports Coordinator

Location: 2080 Cabot Blvd. West, Langhorne, PA 19047

FT/PT: Full-time

General description/ Qualifications:

Minimum Qualifications for Supports Coordinators:

A bachelor’s degree, which includes or is supplemented by at least 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social science*.

Bachelor’s degree with 12 eligible credits and no experience

OR

Two years’ experience as a County Social Service Aide 3 and two years of college level course work, which include at least 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social service*.

Two years of college (including earning an associate degree) with 12 eligible credits AND two years’ experience in a position performing paraprofessional case management functions**)

OR

Any equivalent combination of experience and training which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social service* and one year of experience as a County Social Services Aide 3 or similar position performing paraprofessional case management functions**.

12 eligible credits AND three years’ experience in a position performing paraprofessional case management functions**- experience can be calculated through the completion of acceptable social science/social service courses at the rate of 30 credits equaling one year experience.

*Courses that fall under “other related social science/services” include:

Administration of Justice History

Anthropology Human Behavior

Art Therapy Human Development

Child Development Human Services

Counseling Movement Therapy

Criminology Music Therapy

Economics Political Science

English Social Welfare

Geography Special Education

**Definition of paraprofessional case management functions:

Positions in public or private social services agencies, performing activities that includes interviewing; providing information; referring to community resources; and determining or verifying eligibility for services. Direct care such as Therapeutic Staff Support, Personal Care Assistants, Direct Support Professional etc. does not meet the requirement of paraprofessional case management.

Must have a valid driver’s license prior to being offered the position.

Duties/Responsibilities:

The Intellectual Disabilities Supports Coordinator provides case management and service coordination to a caseload of 36-42 clients.

The Intellectual Disabilities Supports Coordinator, with the input of the individual served and their support network, develops an individualized service plan that outlines the individual’s services and supports and documents the individual’s health and safety needs.

The Intellectual Disabilities Supports Coordinator works with complex individuals who may have co-occurring or dually diagnosed clinical issues.

The Intellectual Disabilities Supports Coordinator provides on-site monitoring of individual’s health, safety and services per state regulations.

The Intellectual Disabilities Supports Coordinator interfaces with state and county representatives, families, providers, social service organizations, mental health providers, behavioral support providers and at times probation/parole and the criminal justice system.

The Intellectual Disabilities Supports Coordinator is responsible for maintaining the record of their individuals’ diagnoses, medications, health concerns needing health promotion recommendations, supervision care needs and service needs.

The Intellectual Disabilities Supports Coordinator assists individuals in assessing their eligibility for services, prioritizing their need for services and linking individuals to service providers.

Equipment use including but not limited to computer (including repetitive motions of typing, seeing monitor), copier, fax, printer, shredder, telephone, cell phone, and voice mail and light lifting up to 10 pounds, ability to drive agency vehicles.
