The Fatherhood Coach works hand in hand with healthcare providers and community health workers to ensure compliance with UH PNP and Healthy Start requirements and functions as an integrated member of the All Babies Initiative (allbabies.org). The Fatherhood Coach specializes in connecting with and engaging fathers and partners to promote healthy family involvement and relationships. This role provides evidence-based education, develops and monitors patient care plans, and conducts outreach to ensure that social, financial, nutritional, and family wellness needs are identified and addressed.

Fatherhood Coach Essential Job Duties:

• Collaborates with partner clinics and social services providers to ensure resources are available to families.
• Communicate with fathers to ensure provider connectivity and appointment availability.
• Assist fathers in their homes, community, or clinic setting. Communicates to clients/patients the purpose of the program and the impact it may have on their wellbeing.
• Helps fathers identify socio-economic issues that affect their overall health and develop health/social management plans and goals.
• Follows established guidelines to provide consistent, evidence-based patient education regarding safe sleep and best choice infant nutrition practices.
• Documents and maintain records of patient interactions through ChallengerSoft Case Management system; maintains comprehensive electronic client files, which include client notes, release of information, assessments and other medical documents acquired on behalf of the client. Documents activities, services plan and outcomes achieved by the client in an effective manner.
• Monitor and track progress and establish intervention when care compliance is not consistent.
• Complies with Health Insurance Portability and Accountability Act standards.
• Assist with planning, preparation, and providing parent groups, effectively delivering father focused groups and events.
• Coach patients in effective management of their chronic health conditions and self-care. Assist patients in understanding care plans and instructions. Motivates patients/clients to be active and engaged in their health overall wellbeing.
• Participate in community events, work groups, and committees to promote responsible fatherhood and the importance of fathers' contribution to the safety, permanency, and well-being of children.
• Build professional relationships with expectant fathers and families.
• Encourages families to attend childbirth, parenting, and family education classes.
• Ensure fathers-to-be are well informed and understand prenatal care plans.
• Help fathers and families remove barriers to prenatal care.
• Provides education, guidance, and resources on health-related services, overcoming barriers to obtaining needed medical care and /or social services.
• Assist families to apply for social services, including but not limited to: publicly sponsored health care insurance plans, or Women, Infants, and Children (WIC).
• Travels extensively to client homes, community locations, various agencies, and other outreach destinations.
• Establishes positive, supportive relationships with participants and provides feedback to other members of the team. Builds and maintains positive working relationships with the clients, providers, nurse case managers, agency representatives, supervisors and office staff, from diverse cultural and socio-economic backgrounds. Works to reduce cultural and socio-economic barriers between clients and institutions.
• Support peers in utilizing effective strategies to engage fathers in program services and community resources, including but not limited to, metal illness, substance abuse, housing, employment, etc. that affect family well-being.
• Provide parenting education utilizing evidence-based treatment models and curriculum.
• Assists clients in accessing health-related services, including but not limited to: obtaining a medical home, providing needed medical care and/or social services.
• Provides support and advocacy during initial medical visit or when necessary to assure clients' medical needs and referrals required are being conveyed. Follows up with both clients and providers regarding health/social services plans.
• Performs miscellaneous job-related duties as assigned.

Education, Training & Experience

• Bachelor’s degree in medical field, social services, or health services required.
• Excellent oral and written communication skills, proven critical thinking skills, excellent computer skills, knowledge of multiple computer programs, and ability to multi-task.
• Applicants must have a working vehicle to successfully facilitate transportation to/from but not limited to: home visits, community visits, and clinic sites.
