Role Overview
The Patient Services Coordinator serves as a cross-functional team member responsible for providing exceptional patient service through both administrative and clinical support functions. This position ensures efficient clinic operations by coordinating patient scheduling, registration, intake, care support activities, and provider support while promoting a positive patient experience and compliance with organizational standards.
Essential Duties and Responsibilities
1. Patient Registration and Scheduling
- Greet and assist patients in a professional and courteous manner.
- Perform patient check-in and check-out functions.
- Schedule, reschedule, and confirm appointments.
- Verify patient demographics, insurance, and eligibility information.
- Manage incoming calls and patient inquiries.
2. Clinical Support and Patient Intake
- Prepare patients for examinations and procedures.
- Obtain and document vital signs, medical histories, and chief complaints as appropriate.
- Assist providers during patient visits and clinical procedures.
- Maintain accurate documentation within the electronic health record (EHR).
- Support care coordination and follow-up activities.
3. Patient Experience and Customer Service
- Provide exceptional customer service to patients, families, and visitors.
- Educate patients regarding clinic processes, appointments, and referrals.
- Address patient concerns and escalate issues appropriately.
- Promote a welcoming, patient-centered environment.
4. Care Coordination and Administrative Support
- Coordinate referrals, authorizations, and follow-up appointments.
- Assist with completion of forms, records requests, and patient communications.
- Monitor provider schedules and support clinic workflow.
- Facilitate communication between patients, providers, and community resources.
5. Compliance, Quality, and Operational Support
- Adhere to organizational policies, procedures, and regulatory requirements.
- Maintain confidentiality of patient information in accordance with HIPAA guidelines.
- Participate in quality improvement initiatives and workflow enhancements.
- Maintain exam rooms, front office areas, and supplies necessary for clinic operations.
- Support organizational goals related to patient access, quality, and patient satisfaction.
6. Mobile Clinic Operations
- Assist with setup, breakdown, and daily readiness of the mobile clinic.
- Load, unload, organize, and secure medical equipment, supplies, medications, vaccines, and outreach materials.
- Maintain inventory levels and communicate supply needs.
- Participate in community outreach events and mobile clinic activities.
- Support registration, patient flow, and clinical operations at community locations.
- Ensure the mobile clinic remains clean, organized, safe, and operationally ready.
- Adapt workflows and responsibilities based on patient volume, location, and operational needs.
- Ability to drive Mobile Clinic as needed.
- Represent Avenue 360 professionally in community settings and partnership events.
7. Quality, Compliance Sterilization & Safety
- Adhere to OSHA, HIPAA, infection control, and patient safety regulations.
- Follow organizational policies regarding medication and vaccine management.
- Complete required quality assurance and quality improvement activities.
- Participate in incident reporting, emergency preparedness, and safety protocols.
- Maintain knowledge of Ryan White, Title X, Sliding Fee Scale, TVFC, and other applicable program requirements.
- Complete all required organizational training, certifications, and continuing education.
- Clean, sterilize, and maintain dental instruments and equipment.
- Follow infection prevention and control protocols.
- Adhere to OSHA, radiology, Joint Commission, and agency standards.
- Support quality improvement and compliance initiatives.
- Maintain accurate documentation and regulatory compliance.
8. Flexibility & Additional Responsibilities
- Perform their duties as assigned to support clinic operations and organizational needs.
- Adapt to changing priorities and assist with tasks outside of routine responsibilities when necessary.

Experience, Skills & Qualifications
Required
- High School Diploma or GED required.
- Medical Assistant Certification, Dental Assistant Certification, or equivalent healthcare experience preferred.
- Minimum one (1) year of experience in a healthcare or clinical setting preferred.
- Knowledge of electronic health records and medical office operations.
- Strong communication, customer service, and organizational skills.
- Good driving record.
Work Environment & Physical Requirements
- Work is performed within a mobile healthcare unit, clinic setting, and community-based locations.
- Frequent local travel between Avenue 360 locations and community outreach sites is required.
- May work outdoors and be exposed to varying weather conditions including heat, humidity, rain, and cold.
- May be exposed to bloodborne pathogens, infectious diseases, and other healthcare-related hazards.
Physical Requirements
The employee must be able to:
- Frequent sitting and standing, occasional walking, and the ability to perform routine physical activities associated with front office and dental clinical support.
- The role may require lifting up to 25 pounds occasionally and maintaining prolonged periods of activity throughout the workday.
- Frequently enter and exit the mobile clinic vehicle.
- Lift, carry, push, and pull medical supplies, vaccine coolers, equipment, and outreach materials weighing up to 35 pounds independently and heavier items with assistance.
- Bend, squat, stoop, kneel, reach, and perform repetitive motions.
- Load and unload clinic supplies and equipment.
- Sit for extended periods while performing documentation and administrative work.
- Demonstrate the manual dexterity necessary to administer injections, perform phlebotomy, specimen collection, EKGs, and other clinical procedures.
- Maintain visual acuity and attention to detail required for patient care, medication administration, and documentation.
