JOB SUMMARY:
The Program Specialist serves as a vital member of Trenton Health Team (THT), balancing project leadership responsibilities with active community engagement. This role drives project success by managing timelines, tracking metrics, and building strong relationships with community stakeholders to advance public health and clinical operations in the Trenton area. While project areas may vary over time, this role will primarily focus on on THT’s Substance Use and Medicaid Supports Program Areas, playing a critical role in creating a community where those who use drugs are able to do so safely and where everyone who is eligible for Medicaid is able to get access and use their benefits.
ESSENTIAL FUNCTIONS:
Project Leadership: Serve as project lead on a selected number of projects to include:
Team Coordination: Convene project teams consisting of THT staff and external partners; clearly define and assign roles and responsibilities.
Project Governance: Develop and maintain project charters, work breakdown structures, and other tools in consultation with the Project Supervisor.
Metrics & Outcomes: Partner with the project team to define, develop, and implement systems to track and measure project outcomes and metrics.
Documentation: Create and maintain project implementation folders within the shared drive, ensuring all drafts, final products, and relevant documents are organized and securely stored.
Communication: Facilitate internal team communications and lead productive meetings.
Progress Management: Manage team workflows to ensure timely progress toward milestones, deliverables, and KPIs. Actively consult with the Project Supervisor regarding project status or roadblocks.
Budget Management: Monitor spending on projects and approve monthly spending and variance reports.
Community Engagement
Participates in stakeholder meetings on behalf of THT.
Attends,organizes, and facilitates community meetings, health fairs and other events to build relationships with community members and organizations.
Keeps abreast of important community news- identifying barriers, challenges, and facilitators to programs - and building new partnerships.
ADDITIONAL FUNCTIONS:
Supports other THT programs and activities as needed.
Provides periodic verbal and written reports regarding the status and progress of work.
Participates in and contributes to regular team meetings
Assumes all other duties and responsibilities as assigned

REQUIRED QUALIFICATIONS
Bachelor’s degree, or equivalent combination of education and experience.
Professional experience in community-based work (including outreach and engagement) and/or public health or clinical operations or administration.
Strong organizational, problem-solving, and analytical skills; able to manage priorities and multiple deadlines.
Commitment to serving low-income communities.
Versatility, flexibility, and a willingness to work within constantly changing priorities with enthusiasm.
Excellent interpersonal skills with ability to communicate and interact effectively with individuals from diverse groups regardless of race, sex, age, and/or sexual orientation and members of underserved communities.
Excellent presentation skills with an ability to motivate and energize community members/groups to action.
Demonstrates excellent written and verbal communication skills with the ability to communicate across all strata of the community.
Excels working in a dynamic environment.
PREFERRED QUALIFICATIONS
Experience working in Trenton communities.
Bilingual in written and spoken English and Spanish.
COMPUTER SKILLS
Excellent computer skills with specific knowledge of Google Workspace, Microsoft Office programs, and web-browsers/web-based programs; Experience with Salesforce, Constant Contact or similar tools is preferred.
