Title VI Elder Care Provider (ECP) in Perkins, Oklahoma at Iowa Tribe of Oklahoma

The Title VI Elder Care Provider (ECP) acts as an advocate and facilitator for Title VI Elders to ensure access to comprehensive support and daily well-being services. This position delivers a continuum of services through nutrition and self-care education, social support services, well-being monitoring, daily activities, transportation, meal assistance, chore services, and hygiene care. The role delivers service coordination, connects elders to vital resources, and provides direct assistance within community-based settings, including elders’ private homes. This position requires deep cultural respect to promote a welcoming environment that reflects the Tribe’s culture, vision, and values.

DUTIES/RESPONSIBILITIES:

• Assist elders in their homes or community spaces in strict accordance with program guidelines.
• Communicate the core purposes of the Title VI program to elders and explain the positive impacts it can have on their overall well-being.
• Complete program intake forms and document all elder encounters while keeping accurate records, maintaining strict confidentiality, and respecting privacy.
• Identify gaps in services and coordinate with program staff to address issues affecting the elders' daily social and nutritional needs.
• Work collaboratively with the program team to help elders develop individual personal well-being or social goals based on intake findings.
• Serve as a culturally informed liaison and advocate for the elderly population within the tribal community.
• Provide culturally appropriate, accurate education to elders, families, and community organizations to promote healthy behaviors and independence.
• Coach elders on effective wellness and self-care practices while helping them understand specific program options and instructions.
• Motivate elders to remain active, engaged, and socially connected in their daily lives to support their long-term well-being.
• Provide support and advocacy during community visits when necessary to ensure the elder's personal needs are effectively met.
• Provide or arrange transportation services and resource deliveries for elders following established program policies and procedures.
• Perform light chore duties and assist with daily personal hygiene care inside the elders’ homes.
• Collaborate with other tribal departments to ensure elders utilize all available tribal and community social services.
• Create, organize, and implement elder-friendly daily activities and community events.
• Assist in meeting daily nutritional needs by actively participating in meal planning, meal preparation, meal service, and home-delivery meal routes.
• Complete and submit accurate monthly program reports to the supervisor.
• Perform other duties as assigned.

REQUIRED SKILLS/ABILITIES:

• Ability to provide culturally competent care and apply an understanding of tribal traditions and elder respect.
• Strong decision-making, problem-solving, and critical thinking abilities, particularly in community-based settings.
• Ability to respond appropriately, calmly, and effectively during a personal or building crisis.
• Excellent team-oriented collaboration skills to work across multiple tribal and social departments.
• Knowledge and understanding of program policies, safety compliance, and operational procedures.
• Ability to follow instruction precisely and communicate clearly with elderly individuals and their families.
• Must possess a valid driver’s license and maintain insurability under the Tribe’s vehicle insurance policy to operate program vehicles for elder transportation, meal delivery, and home visits.
• Ability to obtain Cardiopulmonary Resuscitation (CPR), First Aid, and Automated External Defibrillator (AED) certification within six months of employment.
• Ability to document service delivery accurately, compile daily encounter records, and organize information cleanly to assist with required monthly program reporting.
• Ability to obtain a valid Food Handlers Certification within the timeframe established by program guidelines.

EDUCATION & EXPERIENCE:

• High School Diploma or General Educational Development (GED) equivalence required.
• Must be at least 18 years of age and possess good moral character.
• Must possess and maintain an active Oklahoma Certified Nurse Aide (CNA) certification at the time of hire.

PHYSICAL REQUIREMENTS:

• Ability to frequently bend, stoop, kneel, crouch, and sit on the floor to provide direct care or activities in residential or community settings.
• Physical strength to occasionally lift, push, pull, or move objects or equipment weighing up to 50 pounds.
• Stamina to travel regularly and drive program vehicles to transport elders, run meal routes, or complete home visits.
• Manual dexterity to administer basic first aid, assist with personal hygiene, and perform light household chores or meal prep.

CONDITIONS OF EMPLOYMENT:

• Successful completion of applicable tribal drug test.
• Successful completion of applicable tribal background check.
• Must pass background investigation for suitability of character identified in accordance with the Indian Child Protection and Family Violence Prevention Act.
• Possess and maintain a valid driver's license with no serious violations.
