This position is listed on behalf of a partner company, who manages all applications and next steps. Our partner is looking for a Community Health Worker - Medicaid based in the United States.
This role serves as a trusted connection between Medicaid members, healthcare providers, community organizations, and care management teams.
You will work directly with members to improve health literacy, address barriers to care, and connect people with valuable community resources.
The position combines face-to-face and telephone outreach with education, advocacy, care coordination, and social needs support.
You will focus on helping members close care gaps, access preventive services, and take a more active role in their health and wellness.
The role also provides an opportunity to build strong partnerships with community organizations serving vulnerable and at-risk populations.
You will work independently while collaborating closely with internal care management teams and external providers.

- Conduct face-to-face and telephone outreach to Medicaid members around preventive health, care gaps, maternity, care transitions, health-related social needs, and chronic condition initiatives.
- Educate members on the importance of preventive healthcare services, assess opportunities for additional care management support, and make appropriate referrals to internal programs and community resources.
- Coordinate with members, healthcare providers, and community organizations to help individuals access preventive services and address health-related social needs.
- Build trusting relationships with members and empower them to take an active role in managing their health, wellness, and healthcare decisions.
- Develop partnerships with community-based organizations and human services providers, including programs such as Head Start, WIC, community centers, and homeless shelters, to identify and engage members who may need additional support.
- Conduct Social Determinants of Health assessments when appropriate and help members address barriers such as transportation, childcare, financial challenges, limited healthcare knowledge, and difficulty navigating the healthcare system.
- Assist members with scheduling appointments, answering healthcare-related questions, and connecting them with appropriate services and resources.
- Develop and contribute to educational materials for members and providers and communicate outreach results, outcomes, and community education activities to management and colleagues.
- Participate in developing measurable and innovative programs designed to reach and support at-risk member populations.
- Maintain current knowledge of available health plan benefits and lines of business to ensure members receive accurate and relevant information.
- Represent the organization at community events and activities hosted by community partners.
- Collaborate closely with care management and other internal teams to refer members for additional education, support, and services outside the role’s scope.
- Have at least 1 year of experience in a healthcare-related customer service, outreach, or marketing environment demonstrating the ability to engage members and encourage participation in health or care management programs.
- Have at least 1 year of experience as a Community Health Worker or in a closely related field.
- Hold a High School diploma or GED.
- Obtain Certified Community Health Worker (CCHW) certification through the Pennsylvania Certification Board within 12 months of the start date.
- Demonstrate knowledge of customer service principles, including understanding customer needs and maintaining appropriate quality standards.
- Communicate effectively with members, peers, supervisors, providers, and community partners while contributing positively as part of a team.
- Demonstrate independent and creative problem-solving skills, with the ability to be proactive, self-directed, assertive, and resourceful.
- Be able to manage multiple priorities simultaneously and respond promptly and professionally to member needs and concerns.
- Possess strong written and verbal communication skills, along with patience, empathy, and a member-focused approach.
- Demonstrate cultural awareness and sensitivity when working with Medicaid populations, including individuals with limited English proficiency and diverse social and cultural backgrounds.
- Understand common healthcare access barriers, including transportation, childcare, limited preventive-health knowledge, distrust of healthcare systems, and personal, social, or financial challenges.
- Demonstrate the ability to work independently in the community while maintaining strong collaboration with internal and external stakeholders.
- Basic medical terminology knowledge is preferred.
- Experience working directly with the targeted community or population is preferred.
- Be prepared to travel approximately 50%–75% as required by the role.
- Competitive base pay ranging from $20.72 to $30.12 per hour, depending on qualifications, experience, expected contributions, internal equity, market factors, and business considerations.
