Rehab Director- Physical Therapist Assistant in Dyersburg, Tennessee at Oakwood Community Living Center

Oakwood Community Living Center is seeking an experienced and motivated Rehab Director- Physical Therapist Assistant to lead our therapy department while providing high-quality rehabilitative services to our residents.

• Oversee daily operations of the rehabilitation department.
• Assist in developing and implementing individualized treatment plans under the direction of the Physical Therapist.
• Provide therapeutic exercises and activities to residents.
• Monitor, document, and communicate resident progress.
• Ensure accurate and timely EMR documentation.
• Educate residents and families regarding treatment plans and therapeutic equipment.
• Coordinate therapy services with interdisciplinary team members.
• Maintain compliance with all facility, state, and federal regulations.
• Foster a culture of teamwork, collaboration, and resident-centered care.
• Ensure a safe, clean, and organized therapy environment.
• Perform additional duties as assigned.

• Graduate of an accredited PTA/OTA program.
• Current PTA/OTA license in the state of employment.
• Previous leadership or supervisory experience preferred.
• Skilled Nursing Facility (SNF) or rehabilitation experience preferred.
