Position Summary
The Spiritual Care Provider delivers compassionate, patient-centered spiritual support to individuals and families receiving hospice services through Snowline Health. This role helps patients honor their personal beliefs, values, and end-of-life goals while promoting dignity, meaning, comfort, and peace throughout the hospice journey.
Working as a collaborative member of the interdisciplinary care team, the Spiritual Care Provider assesses spiritual needs, develops care plans, provides counseling and guidance, and supports patients and families as they navigate illness, loss, grief, and end-of-life transitions. The ideal candidate demonstrates exceptional empathy, cultural humility, effective communication skills, and a deep respect for diverse faith traditions, spiritual practices, and personal beliefs.
Responsibilities Include:
- Honor patient wishes and support them in achieving their spiritual goals
- Help patients leave this world with dignity, comfort, and peace
- Assess spiritual needs of patients, families, and caregivers related to the terminal illness process
- Complete spiritual assessments, documentation, and care plans in accordance with organizational and regulatory requirements
- Provide spiritual counseling and emotional support related to suffering, loss, grief, hope, and meaning
- Respect and support diverse cultural, spiritual, and religious beliefs, values, and traditions
- Collaborate with the interdisciplinary team to ensure patient comfort and quality care
- Integrate spiritual needs and interventions into the patient's overall plan of care
- Evaluate the effectiveness of spiritual interventions and implement follow-up actions as needed
- Educate and consult with team members regarding spiritual concerns and end-of-life issues
- Participate in team meetings, trainings, and community events
- Maintain timely and accurate documentation according to organizational standards
- Demonstrate professionalism, initiative, adaptability, and dependability
- Perform additional duties as assigned
- Education in world religions, spiritual traditions, and cultural practices preferred
- Religious Science Practitioner certification preferred
- Master's Degree in Divinity, Theology, or related field from an accredited college, university, or seminary preferred
- Minimum of two (2) years of experience working with patients and families facing life-threatening illness, death, and grief
- Previous healthcare, hospice, pastoral care, chaplaincy, or spiritual care experience preferred
- Four (4) Clinical Pastoral Education (CPE) units preferred
- Strong verbal, written, and interpersonal communication skills
- Demonstrated understanding and appreciation of diverse faith traditions and spiritual beliefs
- Ability to provide compassionate support in emotionally sensitive situations
- Ability to efficiently operate a computer; Electronic Health Records (EHR) experience preferred
- Valid California driver's license and a reliable, insured vehicle
- Comfortable being mobile and traveling throughout the service area
- Open to an occasional flexible schedule, including evenings or weekends
