Job Overview
We are seeking an experienced Home Health Physical Therapist (PT) to join our growing team. We are looking for a skilled, dependable clinician who is comfortable working independently in patients' homes and understands the unique clinical and documentation requirements of Medicare-certified home health.

Requirements:

What You’ll Do

  • Perform comprehensive physical therapy evaluations and assessments in patients' homes.
  • Develop individualized treatment plans based on patients' functional needs and goals.
  • Provide skilled physical therapy services, including therapeutic exercises, mobility training, balance and gait training, strengthening, and other appropriate interventions.
  • Monitor patient progress and modify treatment plans as needed.
  • Educate patients and caregivers regarding exercises, safety, mobility, fall prevention, and home programs.
  • Communicate changes in patient condition and progress with physicians, the clinical team, and other members of the care team.
  • Coordinate care with the RN case manager, PTAs, OTs, SLPs, physicians, and other healthcare professionals.
  • Participate in discharge planning and help patients achieve the highest possible level of functional independence.
  • Complete all required clinical documentation accurately and within required timeframes.
  • Follow Medicare, Illinois, and agency requirements for home health services.
  • Provide supervision and guidance to Physical Therapy Assistants when applicable.
  • Participate in agency quality improvement, training, and competency activities.

Home Health Experience and Technology SkillsHome health experience is strongly preferred. The ideal candidate understands that home health is different from outpatient or facility-based therapy and is comfortable managing a caseload independently.Experience with home health EMR and technology systems is highly valued, including:

  • Home health EMR documentation and point-of-care systems
  • Electronic visit scheduling and visit verification/EVV systems
  • Electronic physician orders and care-plan management
  • Electronic clinical documentation and communication
  • OASIS-related documentation and the Medicare home health workflow
  • Understanding of PDGM, Medicare Conditions of Participation, and home health documentation requirements
  • Ability to learn and efficiently use new home health software and technology
  • Microsoft Office, including Word, Excel, and Outlook

What We’re Looking For

  • Current, unrestricted Illinois Physical Therapist (PT) license
  • Previous home health PT experience preferred
  • Knowledge of home health clinical documentation and regulatory requirements
  • Ability to perform comprehensive assessments and develop appropriate plans of care
  • Strong clinical judgment and attention to detail
  • Excellent communication and organizational skills
  • Ability to work independently while remaining an effective member of a multidisciplinary team
  • Dependable, professional, and patient-focused
  • Ability to manage a schedule and complete documentation within required timeframes
  • Valid driver's license, reliable transportation, and willingness to travel to patients' homes
