REQUIRED:

POSSESSES CURRENT ILLINOIS PHYSICAL THERAPY LICENSE, WEBSITE LISTING, OR LETTER TO TEST FROM DEPARTMENT OF PROFESSIONAL REGULATION IN STATE OF ILLINOIS.
