Job Summary

The Direct Support Professional reports directly to the assigned HCBS Coordinator. Assists the consumer in daily living skills such as self care, independent living, and/or self direction. Assists sand supports the consumer in a manner to allow the consumer to be a participating member in his/her family and community.

Job Duties and Responsibilities

Implements and monitors Person Centered Service Plan

Perform custodial care as needed for the consumer

Maintain behavior management techniques needed

Transport the consumer in the community to non-medical activities

Maintain written daily progress notes

Maintain records for the consumer’s Person Centered Service Plan

Must have the ability to work in a constant state of alertness to ensure the health, welfare, and safety of developmentally disabled persons

Other duties as assigned

Knowledge, Skills and Abilities

Ability to monitor and record daily progress of the consumer

Ability to instruct the consumer in development skills

Ability to maintain a working relationship with the consumer and/or the family

Ability to express thoughts and ideas in writing

Minimum Qualifications

High school diploma/GED

Valid Arkansas Driver’s License

Must be able to provide transportation to the consumer
