About the Role
Hospice & Palliative Care, Inc. is seeking compassionate, skilled, and experienced Registered Nurses (RNs) to join our dedicated team. As a Hospice RN, you’ll play a vital role in delivering quality end-of-life care to patients in the comfort of their homes while providing emotional and clinical support to families.
This is a full-time, community-based position with a manageable caseload and a consistent weekday schedule—no weekends, holidays, or on-call requirements.
What You’ll Do
- Provide skilled nursing care, hands-on support, and care coordination for hospice patients.
- Implement, evaluate, and revise individualized care plans in collaboration with the interdisciplinary team and physicians.
- Maintain accurate documentation including progress updates, medication changes, and patient contacts.
- Supervise and delegate care responsibilities to LPNs as appropriate.
- Support patients and families through compassionate communication, education, and clinical expertise.
- Participate in bi-weekly interdisciplinary team meetings and case conferences.

Qualifications
- Current RN license in New York State (required)
- Valid NYS driver's license and reliable transportation (required)
- Associate degree in Nursing (required)
- Minimum two years of RN experience preferred (hospice, home care, hospital, oncology, or long-term care)
- CPR certification (required)
- Strong communication, critical thinking, and clinical assessment skills
