Job Description
POSITION SUMMARY
- Work with healthcare professionals to promote diabetes self-management and contribute to the improvement of patient outcomes. Primary responsibility will be to provide education, support, and resources to patients and their families to empower them to make informed decisions about their diabetes care.
EDUCATION, CREDENTIALS, TRAINING, and EXPERIENCE
- Board certification in Diabetes Care & Education (CDCES), or board eligible, with plan for certification within one year of hire. Eligible disciplines included Registered Nurses, Pharmacist, Physical Therapist, Physician, Dieticians.
- Understanding of cultural diversity and its impact on diabetes care
- Strong interpersonal and communication skills to effectively interact with patients, their families, and healthcare professionals
- Knowledge of diabetes management principles, treatment options, and lifestyle modifications
- Proficiency in using electronic health record systems and basic computer skills
- 2 years experience working with patients with diabetes
ADDITIONAL SPECIALIZED KNOWLEDGE/SKILLS
- Working knowledge of healthcare insurance.
- Strong customer service skills.
- Knowledge of community resources.
- Educates patients on the necessary lifestyle changes involved in a diabetes diagnosis, including nutrition, medication, physical activity, and glucose testing.
- Familiarity with continuous glucose monitoring.
