About the Role:
The Community Health Worker (CHW) will participate/integrate into Minnie Hamilton Health Systems (MHHS) clinics existing Chronic Disease Care Management Program as part of the care management team. The focus is to assist high-risk chronic disease patients with care coordination/self-management skills to improve patient outcomes. Interaction takes place with patients through patient appointments, telephonic calls, home visits and community events and programs.
CHW's are the community-based component of the chronic care management team who work closely with identified groups of patients providing in-home education, motivation for self-management skills and facilitate communication between the patient and their provider and care team.

Responsibilities:
- Conduct outreach to community members to provide health education and promote wellness programs.
- Assist individuals in navigating healthcare systems, including scheduling appointments and understanding treatment plans.
- Identify social determinants of health affecting community members and connect them with relevant social and medical services.
- Collaborate with healthcare providers and community organizations to develop and implement health promotion strategies.
- Maintain accurate records of client interactions and report on community health trends to inform program development.
Preferred Qualifications:
- High School Diploma
- Trusted members of the community being served
- Interpersonal communication skills
- Ability to express empathy and compassion
- Counseling/health promotion skills
- Ability to read and write
- Knowledge of community resources
- Understanding of professional conduct
- Basic electronic/computer skills
- CPR certification
- Proper vital sign measurement
- Valid and unrestricted driver's license, insurance card and willingness to transport patients.
