We are actively seeking a Director of Rehabilitation (DOR) to lead and grow our therapy program in a fast-paced skilled nursing setting. This is a hands-on leadership role for someone ready to take ownership, build a strong team, and drive both clinical outcomes and program growth.

What You’ll Do

  • Lead and manage all therapy services (PT, PTA, OT, SLP)
  • Hire, train, and supervise therapy staff
  • Drive caseload growth and program development
  • Ensure compliance with Medicare, Medicaid, and regulatory standards
  • Oversee scheduling and ensure proper coverage
  • Partner with nursing and IDT to coordinate care plans
  • Participate in care planning, discharge planning, and triple check
  • Monitor outcomes and implement improvements quickly
  • Build relationships with hospitals and referral sources

What You Need

  • Licensed OT or PTA in Oregon
  • Minimum 3 years of rehab/long-term care experience
  • Leadership experience (DOR, ADOR, or Lead Therapist)
  • Knowledge of PPS, PDPM, and skilled nursing operations
  • Strong leadership, communication, and problem-solving skills

Requirements:

  • Current CPR Certification
  • Valid driver’s license
  • Must pass background check
