About This Position

The Mission Neighborhood Health Center (MNHC) is a non-profit, multi-site community health organization offering comprehensive health services in the Mission and Excelsior Districts of San Francisco. The center offers primary health care services, adult medicine, pediatrics, family planning, OB/GYN, HIV and homeless services. We advocate for health equity and deliver innovative, high-quality services responsive to the neighborhoods and diverse communities we serve.

We are currently looking for qualified candidates for our Behavioral Health Case Manager - Youth Focused role. This position reports to the Director of Behavioral Health and Substance Use Counseling and is a regular, full Time, Non-Exempt position with a starting hourly rate range of $29.36 to $31.46 with full benefits.

ESSENTIAL FUNCTIONS / RESPONSIBILITIES

Direct Patient Contact (approximately 80-90%):

  • Perform brief assessments to identify patient needs, concerns, and circumstances.
  • Develop action plans in collaboration with patients.
  • Link patients to community services and support networks.
  • Complete psychosocial screenings for patients seeking care at MNHC and refer to relevant community services.
  • Follow up with patients and agencies by phone or in person to address barriers to access.
  • Assist MNHC patients and families with forms for benefits such as CalFresh, disability, housing, transportation, and emergency assistance.
  • Receive referrals and warm hand-offs from Behavioral Health Clinicians and cross-refer to Behavioral Health when warranted.
  • Accompany patients to appointments as needed.
  • Conduct outreach calls to facilitate attendance at groups and events.
Administrative - Data Reporting, Program Planning & Evaluation (approximately 10-20%):
  • Chart on all patients seen in the Electronic Health Record.
  • Develop, update, and maintain list of resources based on patient's needs (i.e., legal, food, housing, vocational).
  • Participate in supervision meetings, team meetings, and other required health center meetings.
  • Collects and analyzes information before making decisions; seeks input when appropriate.
  • Perform other duties as assigned.

  • Ability to provide support, advocacy, and build relationships following harm reduction principles with a diverse population, including Spanish-speaking immigrants, day laborers, women, LGBTQ+ individuals, African Americans, active drug users, and people with mental illness and other disabilities.

MINIMUM QUALIFICATIONS

  • Associate's degree in social work, Psychology, Counseling or a related field.
  • Substitution for education: three years of paid work experience in a community or healthcare setting.
  • Bilingual in English and Spanish.
PREFERRED QUALIFICATIONS
  • Experience working with youth and adults.
  • Proficiency in Epic EHR.
  • Bachelor's degree.
