POSITION TITLE: | Primary Care Physician |
POSITION SUMMARY: | The Primary Care Physician is responsible for providing comprehensive, patient-centered medical care to individuals and families across the lifespan, with a focus on prevention, chronic disease management, and health equity. The role includes diagnosing and treating acute and chronic conditions, performing routine exams, coordinating care with interdisciplinary teams, and addressing social determinants of health that impact patient outcomes. Physicians are expected to deliver culturally responsive care, support access for underserved populations, and contribute to quality improvement initiatives while adhering to evidence-based practices and regulatory standards. |

Key Responsibilities
Clinical Leadership & Supervision
- Provides clinical supervision of prescribing practices for assigned Advanced Practice Providers (APPs) in accordance with Massachusetts regulations.
- Maintains appropriate availability for consultation and oversight of assigned APPs in accordance with state regulations and organizational policy.
- Models professional standards of care, documentation, patient communication, and teamwork.
- Promotes a collaborative, respectful, and accountable clinical culture.
Direct Patient Care Responsibilities
Maintains an active patient panel consistent with organizational access, panel size, and productivity expectations and provides the full scope of preventive and primary care services within professional licensure and scope of practice, including:
- Establishes and maintains therapeutic relationships with patients.
- Ensures informed consent and patient understanding of diagnoses and treatment plans.
- Provides chronic disease management and preventive care consistent with evidence-based guidelines.
- Integrates assessment of social determinants of health into care planning and collaborates with care teams to address barriers to care.
- Supports access initiatives, including same-day care and panel management strategies.
- Refers appropriately to internal programs (dentistry, behavioral health, diabetes education, OBAT, SRH, etc.) and external specialty providers.
- Works within a team-based care model, collaborating with nursing, care management, behavioral health, and support staff to optimize patient outcomes.
- Documents all encounters in a timely, accurate manner reflecting medical necessity and supporting appropriate coding and billing.
- Submits billing documentation within required timeframes (within 48 business hours of visit).
- Participates in 24-hour on-call rotation as scheduled (approximately four times per year), providing telephone back up to the nurse triage service.
- Transfers care appropriately when patient needs exceed professional scope, skill, or comfort.
Quality Improvement & Compliance
- Participates in departmental and organizational meetings as required.
- Participates in clinical peer review activities in accordance with CHCFC policy and HRSA/FQHC standards.
- Participates in Quality Assurance/Performance Improvement (QA/PI) activities.
- Supports achievement of UDS, HEDIS, and organizational quality benchmarks.
- Engages in targeted improvement efforts when individual or site-level quality metrics fall below benchmarks.
- Reports adverse events, patient safety concerns, or potential risk issues promptly to administration.
- Adheres to clinical protocols and participates in risk management and patient safety initiatives.
- Complies with all CHCFC policies, procedures, and regulatory requirements.
Credentialing & Professional Standards
Maintains all required licensure, certifications, and credentials necessary for clinical practice and payer enrollment as well as:
- Continuing education documentation
Immediately notifies administration of any adverse clinical event, pending litigation, investigation, or disciplinary action by a licensing board.
Other Duties
- Performs other duties as assigned.
- Regular attendance at assigned CHCFC site(s) is required.
Required Qualifications
Education
- Medical degree from an accredited institution (M.D. or D.O.)
Experience
- Primary care experience strongly preferred
Licensure & Certification
- Current Massachusetts license (required)
- Current DEA registration (required)
- Current Massachusetts Controlled Substance Registration (MCSR) (required)
- Board certification (required)
- BLS or ACLS certification (required)

Physical Demands:
The physical demands described here are representative of those required to perform the essential functions of this position, with or without reasonable accommodation. Vision requirements include close, distance, color, peripheral, depth perception, and focus adjustment.
Working Conditions:
This position may require travel between health center locations and exposure to typical weather conditions. Duties may involve exposure to blood, body fluids, or tissues consistent with an outpatient healthcare environment.
