Sylva Valley Health & Rehab is hiring Licensed Practical Nurses (LPNs) for night shift (7p–7a).

RN Pathway Available
We support LPNs who want to grow into RNs with training/schooling opportunities for eligible team members.

• Provide direct nursing care per physician orders and care plans
• Administer medications and treatments safely and accurately
• Monitor residents and report changes in condition
• Document care clearly and timely
• Collaborate with CNAs, nurses, providers, and the interdisciplinary team
• Maintain a safe, respectful, resident-centered environment

• Active Licensed Practical Nurse (LPN) license
• Skilled nursing / long-term care experience preferred
• Strong clinical skills and dependable attendance
• Team-oriented mindset
