Description

Brooklyn Plaza Medical Center’s Behavioral Health Program is committed to providing comprehensive, patient-centered mental health and substance use services to our diverse community. Our team includes licensed mental health professionals, social workers, physicians, and substance use specialists who work together using a Collaborative Care Model. This integrated approach allows behavioral health staff to partner closely with primary care providers to ensure early identification, coordinated treatment, and ongoing support for patients with mental health and substance use needs.

CASAC Responsibilities

Screening and Assessment:
Conduct comprehensive screenings and assessments for Substance Use Disorders (SUD) and co-occurring mental health conditions as part of the patient’s overall medical and behavioral health evaluation.

Treatment Planning:
Work collaboratively with a multidisciplinary team—including physicians, nurses, behavioral health providers, and other clinical staff—to develop and implement integrated, patient-centered treatment plans that address substance use and related medical or psychosocial needs.

Patient and Family Education:
Provide education and counseling to patients, families, and significant others regarding substance use, treatment options, harm reduction, and the recovery process to promote engagement and informed decision-making.

Referrals and Care Coordination:
Coordinate referrals to internal programs and external community-based resources to address patients’ medical, behavioral health, and social support needs. Ensure effective communication with partnering providers to support continuity of care.

Documentation and Record Keeping:
Maintain complete, accurate, and confidential documentation in the electronic medical record (EMR). Ensure all clinical notes, assessments, and billing-related entries are completed in a timely manner to support compliance, quality care, and proper reimbursement.

Professional and administrative duties

  • Regulatory compliance: Adhere to all regulations and standards of both OASAS and the Department of Health, specifically those pertaining to Article 28 facilities.
  • Confidentiality: Strict adherence to federal and state confidentiality requirements, including HIPAA and 42 CFR Part 2, is mandatory.
  • Collaboration: Participate in clinical supervision, staff meetings, and case conferences to ensure effective treatment and communication across disciplines.
  • Toxicology screening: Conduct and manage toxicology testing for patients as needed, following all federal and state regulations.
  • Ethical conduct: Operate in accordance with the CASAC Canon of Ethical Principles and the Justice Center's Code of Conduct.


Qualifications

CASAC Requirements

Bachelor’s Degree (B.A.) required.

New York State Credentialed Alcoholism and Substance Abuse Counselor (CASAC) certification required.

Minimum of one (1) year of experience working with individuals with alcohol or substance use disorders.

Thorough knowledge of addiction treatment principles, procedures, and evidence-based practices from a bio-psycho-social perspective.

Strong interpersonal, written, and verbal communication skills.

Demonstrated ability to work collaboratively as part of a multidisciplinary team.

Genuine commitment to community-based health care and supporting diverse patient populations.
