Family Nurse Practitioner or Physician Assistant 32 Hour Float at SynergenX Health Holdings LLC – Tulsa, Oklahoma

Low T Center, a leader in hormone replacement therapy and targeted wellness, is seeking a Full-Time, 4-Day Float Mid-Level Provider (NP or PA). This is a great opportunity for a provider who values patient-centered care, work-life balance, and being part of a collaborative clinic team.

• Provide hormone replacement therapy, weight management, and wellness care
• Conduct patient assessments and develop individualized treatment plans
• Manage follow-up care and monitor patient progress
• Educate patients on treatment protocols, wellness, and lifestyle optimization
• Collaborate with the Center Director, Medical Director, and clinic staff
• Maintain accurate documentation and ensure compliance with clinic protocols

• Active Oklahoma NP or PA license in good standing (unencumbered)
• Current DEA license
• Current OBNDD license
• Availability to complete a 2-week (10 consecutive business days) training, Monday–Friday during clinic hours

• 32 clinical hours per week
• Clinical shifts: Tuesday, Thursday, Friday, and Saturday
• Clinic hours:
  • Monday: 8 AM – 5 PM
  • Tuesday: 8 AM – 7 PM
  • Wednesday: 8 AM – 1 PM
  • Thursday: 7 AM – 5 PM
  • Friday: 8 AM – 5 PM
  • Saturday: 8 AM – 12 PM

• Salary: $110,000
