About This Position
Job Summary:
The HIM Associate III is responsible to collect, maintain, and make records available in a timely, accurate, and complete manner for all end users in the EMR, as well as those with appropriate authorization to request copies of the medical record. The HIM Associate III is trained on multiple functions of the department and performs any or all of the essential functions for document imaging, analysis, transcription, release of information, and patient portal as is described in this job description and the descriptions for HIM Associate I and II.
Education and Qualifications:
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are the knowledge, skill, and/or ability required.
- High School Degree or equivalent
- Previous experience in the handling of patient health information, medical records document imaging, and/or medical records
Preferred Qualifications:
- Meditech Experience
- Medical Terminology
Essential Duties and Responsibilities:
- Assist patients with Release of Information requests ensuring that all state and federal regulations are followed.
- Handles incoming telephone calls and voicemails from patients, health care professionals, and payors ensuring they are answered appropriately and in a timely manner
- Assists patients with the patient portal, including all questions and user access requests.
- Determines patterns of redundant MRN creation and communicates findings
- Maintains the accuracy, realibability and integrity of EMPI databases
- Monitors timeliness of documentation
- Transcribes dictations meeting TAT and quality standards
- Act as a liason or assist with contracted vendors
- Reviews and uploads H&Ps for upcoming surgeries; provides communication to OR personnel regarding valid H&Ps.
- Monitors and resolves dictation hold queue
- Provides training to associates and providers as necessary. Revises and maintains educational guides as changes occur.
- Performs other related duties incidental to the work described herein
Infection Control: Initial and Ongoing trainings could include but are not limited to, blood borne pathogens, bodily fluids and bio hazardous materials as it applies to your daily work environment.

HIPAA: This position will have access to the following Protected Health Information in order to carry out the duties related to their position at Hays Medical Center based on the following criteria:

Clinical Information (information that describes a patient’s health status): Diagnosis, Reports/Medical Notes, Test Results, Problem List, Procedures, History and Physical
Financial Information/Insurance (information related to insurance, billing and payment): Billing Information, Payer Name, Payer ID, Account Balances, Plan Elements Covered, Payment Information, Payment Rates
Coding Information (clinical information that is in (alpha) numeric format): ICD-9 Codes, Rev Codes, CPT Codes
