Job Description
Immediate start: 12.5 hours a week: 3:30 pm - 6:00 pm, Monday-Friday.
Position Summary:
The Home Health Aide/Caregiver is responsible for delivering high-quality, compassionate in-home care services to patients in accordance with care plans developed by healthcare professionals. This role is essential in supporting clients' daily living activities while ensuring their safety, dignity, and well-being. The successful candidate will provide personal care, assist with instrumental activities of daily living, maintain accurate documentation, and adhere to all applicable state regulations and internal policies.
Key Responsibilities:
Personal Care Assistance:
Assist patients with activities of daily living (ADLs) such as bathing, dressing, grooming, toileting, and mobility support.
Provide safe, respectful, and dignified care, always prioritizing patient comfort and privacy.
Instrumental Activities of Daily Living (IADLs):
Support patients with meal preparation, housekeeping, laundry, and errands.
Ensure the patient's home environment is conducive to recovery and overall well-being.
Health Monitoring and Documentation:
Observe and document changes in patient condition and promptly report any significant findings to your supervisor.
Maintains detailed records of daily care activities, ensuring compliance with Reliable Home Care Providers policies and regulatory guidelines.
Assist with medication reminders - provide support in medication management.
Follow prescribed care plans accurately, ensuring all tasks are completed per state guidelines.
Compliance and Infection Control:
Adhere to infection control protocols and proper use of personal protective equipment (PPE) as mandated by the Illinois Department of Public Health.
Ensure all practices meet the standards of confidentiality, privacy, and patient rights.
Collaboration and Communication:
Provide emotional support and companionship, thereby contributing to the patient's/clients' overall quality of life.
Assist in coordinating additional services or referrals when needed, ensuring continuity of care.
