About Bremen Hospital
Community Hospital of Bremen is a 24-bed critical access hospital serving Bremen, Indiana, and the surrounding communities in Marshall and St. Joseph counties. As part of Beacon Health System, patients have access to providers, resources and services from across the entire health system. The hospital provides the community with: Emergency Department; diagnostic imaging and lab; radiology, including MRI, CT and ultrasound; general surgery; obstetrics; sleep lab; mammography; physical and occupational therapy; occupational health; swing bed services; and infusion therapy.
What You’ll Do
As an inpatient Physical Therapist, you’ll report to the Rehab Services Supervisor. Evaluates patients, plans treatment programs and performs physical therapy procedures to restore function, prevent disability, reduce pain and maximize potential and quality of life following disease, injury or surgery.
Physical Therapist Job Responsibilities

Conduct assessments per physician orders; develop and adjust treatment plans with measurable goals.

Communicate with physicians on therapy changes; ensure prescriptions remain current.

Supervise therapy assistants and maintain compliance with regulatory guidelines.

Initiate discharge planning at evaluation; apply evidence-based techniques for optimal outcomes.

Educate patients and families with clear instructions and supporting materials.

Meet productivity targets; assist team proactively when caseload is light.

Demonstrate flexibility in managing daily workload; participate in meetings and projects.

Ensure compliance with all deadlines, policies, and regulatory standards.

Foster collaboration across disciplines; provide clear direction to support staff.

Complete all evaluations, notes, and discharge summaries within policy timelines.

Ensure documentation supports billing, regulatory compliance, and patient outcomes.

Maintain accuracy and detail in all records; sign/cosign per policy.

Stay current through continuing education; share knowledge via in-services.

Serve as a clinical resource; maintain expertise through literature review and professional engagement.
What You Bring
As a Physical Therapist, you’ll bring a familiarity with use of modalities and equipment needed to complete standard treatments offered. You have tact and discretion when working with patients, families, and physicians and comply with hospital expectations regarding ethical behavior and standards of conduct. You comply with federal and hospital regulations regarding protected health information and patient privacy.
Required Qualifications

Successful completion of a Bachelor's Degree

Physical Therapist licensure in the State of Indiana

State issued Driver's License
-
Previous acute medical care is preferred
