Key Responsibilities:

  • Lead and inspire the Care Management team to deliver exceptional, relationship-based care.
  • Mentor and train Care Managers and Coordinators in best-practice care delivery.
  • Contribute to the Senior Leadership Team with ideas that improve processes and staff wellbeing.
  • Support quality and compliance initiatives across both aged care and disability programs.
  • Manage a client case load to stay connected with the care experience.

About You

A person who thrives in dynamic environments, balancing multiple priorities while maintaining a strong focus on quality, compliance, and client experience. Your communication and problem-solving skills allow you to build meaningful relationships with clients, families, and within your team.

You bring:

  • Strong experience in coordinating and managing home care or disability services.
  • Sound understanding of Aged Care and Disability legislation and compliance standards.
  • Excellent leadership, communication, and relationship-building skills.
  • Experience in budgeting, business management, and stakeholder engagement.
  • Assessment, problem-solving, and innovative care planning
