Position Summary

The Family Medicine Nurse Practitioner will provide comprehensive primary care services in local nursing homes. This position offers a rewarding opportunity to serve patients while also managing care for elderly and long-term care populations.

Responsibilities:

  • Deliver primary care services including health assessments, diagnosis, treatment planning, and disease prevention
  • Manage chronic and acute health conditions in collaboration with supervising physicians
  • Provide continuity of care by rounding at local nursing homes on a scheduled basis
  • Order and interpret diagnostic tests and lab work
  • Educate patients and families on treatment plans and preventive care
  • Document all patient interactions in the EHR
  • Coordinate care with other healthcare professionals and facilities

Qualifications:

  • Current Nurse Practitioner license in the State of Illinois
  • National certification as a Family Nurse Practitioner (FNP-C or FNP-BC)
  • Minimum of 1–2 years of experience in family medicine or primary care preferred
  • Experience or interest in geriatric care and nursing home rounding
  • Strong communication, critical thinking, and organizational skills
  • Ability to work both independently and as part of a multidisciplinary team

Compensation & Benefits:

  • Salary Range: $100,000–$140,000 annually (Collections Based Pay Structure)
