Admission Scheduling Coordinator

Position Profile

Coordinates scheduling admission process for patients in a non-hospital setting. Promotes patient/family centered care through exemplary internal and external customer service.

Responsibilities

§ Coordinates effective and efficient admission process.

§ Manages phone/email communications effectively and efficiently.

§ Processes timely and accurate patient status reports.

§ Creates timely and accurate documentation.

§ Utilizes resources effectively and efficiently.

§ Adheres to scope of practice/HOV policies/standards; facilities continuously improved

processes/services.

§ Maintains community resource information for people in the community.

§ Provides admission clinical resource support. (nurse incumbents only)

§ Maintains and enhances professional skills.

§ Adheres to high standards of personal and professional conduct.

Minimum Qualification

§ High school diploma or equivalent experience.

§ Minimum 3 years medical scheduling/office experience.

§ RN licensure (nurse incumbents only)

Preferred Qualifications

§ Previous hospice/homecare staffing experience preferred.

§ Familiarity with basic medical terminology preferred.
