About This Position

Description:

Specific Responsibilities include, however, are not limited to:

  • Strongly preferred working knowledge of the military health care system, including TRICARE health benefits or fee-for-service principles, preferred provider network, and managed care/HMO organizations, is highly desirable.
  • Serves as part of the Primary Care Manager (PCM) team within the Air Force Medical Home (AFMH) healthcare delivery model. Will provide nursing assistance in the total spectrum management of professional and patient care activities within their scope of practice.
  • Assesses and identifies individual patient care needs. Applies evidence-based practice (EBP) and nursing processes in making appropriate clinical decisions for pediatric, adolescent, adult, obstetric, and geriatric patients.
  • Ability to synthesize data from various sources and make appropriate clinical decisions.
  • Coordinates patient care through a continuum and facilitates the achievement of optimal outcomes concerning clinical care, quality, and cost-effectiveness.
  • Ensures compliance with standards of care and practice per all established policies, procedures, and guidelines used in the medical treatment facility. Provides care within ethical and legal boundaries.
  • Accountable for making patient care assignments based on the scope of practice and skill level of assigned personnel.
  • Excellent patient assessment skills, including the ability to assess/evaluate needs telephonically.
  • Ability to recognize adverse signs and symptoms and quickly react in emergencies.
  • Ability to effectively communicate and collaborate with a diverse group of people to inform the healthcare team of plans/actions for teaching/education to benefit the patient/family and organization. Effectively uses appropriate communication format in addressing professional issues.
  • Excellent oral communication skills for patient/family education and telephone triage of patients. Telephone and in-person assessment/evaluation includes determining the optimal time and location for patient management (ER or clinic) and follow-up care as required. Prescribes and communicates treatment plans and patient teaching per established protocols. Collects and assesses significant patient history information and performs all necessary patient teaching.
  • Excellent written communication skills to perform accurate written and electronic documentation of all activity, including telephone contacts, in accordance with requirements.
  • Makes referral appointments and arranges specialty care as appropriate.
  • Skill in setting up, operating, and monitoring specialized medical equipment such as cardiac monitoring devices, EKG units, oxygen analyzers, nebulizers, and other clinic-specific equipment.
  • Per established protocols, per established protocols, performs/assists with procedures such as bladder catheterizations, administration of oxygen, visual acuity tests, EKGs, irrigation of eyes and ears, tympanograms, splinting, phlebotomy, and other clinic-specific procedures.
  • Ensures proper documentation of procedures and treatments.
  • Ensures all equipment is properly cleaned and stored at the end of each work shift. Ensures preventive maintenance and routine cleaning are performed as directed. Ensures medication/supplies are replenished.
  • Completes orientation and competency verification programs per (IAW) unit guidelines.
  • Participates in the orientation, training, and evaluation of duty performance of newly assigned personnel, as appropriate.
  • Serves/participates in committees, functions, and other meetings as directed. Provides relevant and timely information to these groups and assists with decision-making and evidence-based process improvements. Participates in customer service initiatives and medical readiness activities to enhance health services.
  • Ensures a safe work environment, employee safe work habits, and patient safety per regulatory agencies, infection control policies, and process improvement initiatives.
  • Ensures compliance with professional standards of care and practice (i.e., American Nurses Association (ANA), American Academy of Ambulatory Care Nursing (AAACN), The Joint Commission (TJC), Centers for Disease Control and Prevention (CDC), Occupational Safety and Health Administration (OSHA), etc.). Adheres to Health Insurance Portability and Accountability Act (HIPAA) guidelines, the Department of Defense (DoD) Health Information Privacy Program, and DoD/Defense Health Agency (DHA)/Air Force/Military Treatment Facility (MTF) policies, procedures, and guidelines. Provides care within ethical and legal boundaries.
  • Section flexibility. Contract personnel may be required to work in other MTF duty sections. Contract personnel shall be flexible in rotating temporarily or permanently moving into other duty sections upon notification of the COR. If moved into a new duty section, they will be required to undergo orientation to the new section.
Requirements:

Education and Qualification Requirements:

  • Successfully graduated from a nationally accredited associate's degree or baccalaureate degree program in nursing and holds current certification as a registered nurse (RN)
  • Shall have and maintain an active, valid, unrestricted, current medical license with no limitations, stipulations, or pending adverse actions to practice nursing as a registered nurse in any US State.
  • Knowledge of professional care theories, principles, practices, and procedures to perform nursing assignments of moderate difficulty and experience in assessing adolescent, pediatrics, adults, obstetrics, gynecologic, and geriatric patients, as appropriate. Utilizes the nursing process as a basis for professional practice.
  • As appropriate, knowledge of various medical disorders/conditions and disease processes across the lifespan.
  • Knowledge of various pharmacological agents used in patient treatment, the desired effects, side effects, and complications of their use as well as the accurate administration of the pharmacologic agent, including dosage calculations as required. Knowledge of the administrative requirements for proper documentation of a patient's condition, including disease progress, acknowledgment of teaching, and follow-up care.
  • Knowledge of computer operations and proficiency in basic word processing, data entry, and automated medical records. Proficient in operating standard office equipment, such as personal computers, copiers, fax machines, and telephone systems.
  • Knowledge of medical privacy and confidentiality (Health Insurance Portability and Accountability Act [HIPAA]), The Joint Commission (TJC), and the Clinical Practice Guidelines (CPGs).
  • Must demonstrate ability to communicate effectively both orally and in writing.
  • Must be skillful and tactful in communicating with people who may be physically or mentally ill, uncooperative, fearful, emotionally distraught, and occasionally dangerous.
  • Must have knowledge and experience, or demonstrate comprehension during training, with software and databases currently employed at the MTF (e.g., Microsoft Office, Access, Excel, and PowerPoint; MHS Genesis).
  • Must possess knowledge, skills, and computer program literacy to collect and analyze data.
  • Must have knowledge and experience in Patient Advocacy, Privacy, and Customer Relations.
  • Must possess organization, problem-solving, and communication skills to articulate medical requirements to patients, families/caregivers, and medical and non-medical staff professionally and courteously.
  • Familiarity with the Department of Defense (DoD), Federal, State, and non-profit healthcare systems and organizations, including DoD Tri-Service Military Healthcare Systems, TRICARE purchased care system, Medicare, Medicaid, and the Veterans Administration (VA), is desired.
  • The nurse shall have at least twenty-four (24) months of experience within the last 36 months. This experience shall have been in a primary care clinic setting.
  • Must meet all credentialing requirements for the MTF, where they will provide services.
