Job Description
Responsibilities
The Care Management Associate (CMA) functions as a patient advocate by collaborating with the multidisciplinary care team to facilitate timely and safe transitions of care. The CMA works closely with the care management team as well as the medical and nursing staff to facilitate the coordination of post-acute services.

Opened in November of 2024, this state-of-the-art facility is at 850 South 28th Street and offers comprehensive services, including a 24-hour emergency department, inpatient care, operating rooms and imaging technology. It serves both adults and children. It's designed with the voice of the community in mind and will change health care for generations to come.
Qualifications
