Registered Nurse at Hillcroft Services Inc – Muncie, Indiana

Hillcroft Services is seeking a compassionate and detail‑oriented Registered Nurse (RN) to support individuals receiving Wellness Coordination services. This role plays a key part in ensuring clients receive comprehensive healthcare oversight, accurate documentation, and high‑quality care coordination across multiple providers.

The RN collaborates closely with Case Managers, care teams, residential staff, and contracted pharmacies to ensure each individual’s health and wellness needs are met.

Key Responsibilities

• Evaluate overall health and wellness needs of individuals receiving Wellness Coordination services.
• Monitor daily documentation of client health status
• Complete annual Risk Assessments and collaborate with the interdisciplinary teams to ensure implementation.
• Develop, update, and oversee Wellness Plans annually or as needed.
• Provide training, guidance, and oversight on Medication Administration Records (MAR) for individuals and staff.
• Train staff on Wellness Plans and ensure thorough understanding.
• Conduct consultations with individuals, care teams, and providers according to the individual’s wellness level.
• Participate in annual meetings for individuals receiving Wellness Coordination.
• Attend medical appointments with individuals when needed.
• Review health and wellness documentation during on‑site service delivery.
• Maintain flexible scheduling to meet departmental needs.
• Review quarterly assessments and implement updates or recommendations.
• Maintain all required certifications and participate in ongoing training.
• Provide mentorship to staff as appropriate.

Qualifications

• Active Registered Nurse (RN) license issued by the Indiana State Board of Nursing.
• Experience supporting individuals with developmental disabilities preferred.
• Strong problem‑solving, critical thinking, and teamwork skills.

Additional Requirements

• Ability to pass all applicable background checks, drug screening, and registry clearances.
• Valid Indiana driver’s license, insurable driving record, reliable transportation, and required liability insurance.
• Ability to obtain and maintain agency‑approved First Aid/CPR certification.

• Ability to travel within the community and meet individuals in their homes and transport individuals to appointments as needed.
