PHYSICAL THERAPIST-AZ at Distro – Sun City, Arizona

Position Summary:

• Provide services to restore function, improve mobility, relieve pain, and prevent or limit permanent physical disabilities for residents or patients with injuries or disease.

Essential Duties and Responsibilities:

• Establish and administer treatment plans with specific therapy goals under physician direction.

• Assess patient needs; plan, evaluate, and modify care to meet physical therapy goals.

• Collaborate with all disciplines to plan and evaluate team goals.

• Set realistic and achievable goals for patients/residents.

• Document patient/resident condition and progress per state and federal regulations.

• Educate patients and families on appropriate therapy methods.

• Optimize patient functionality and skills by directing and/or overseeing screening of all residents.

• Communicate patient status and needs to patient, family, and involved professionals.

• Ensure punctuality and regular attendance for assigned shifts.

Other Functions:

• Adhere to Minimum Data Set (MDS) procedures and effective care planning.

• Provide consultation and clinical supervision to Physical Therapist Assistants (PTAs).

• Perform other duties as assigned.

Knowledge, Skills, and Abilities:

• Maintain confidentiality.

• Knowledge of HIPAA.

• General understanding of managed care.

• Knowledge of state and federal therapy delivery and documentation rules in skilled nursing facilities.

Education & Experience:

• Bachelor's, Master's, or Doctorate degree from an accredited Physical Therapy program.

• Current Physical Therapist license in the state of employment.

• Current CPR certification.

• Six months experience in long-term care preferred.
