About Us

ACCORD CARE is a trusted provider of person-centered disability support services to NDIS participants. We deliver Supported Independent Living (SIL), In-Home Care, Community Participation, Short-Term Accommodation (STA), Community Nursing, and Allied Health Services.
About the Role
We are seeking an experienced and compassionate Registered Nurse – Care Coordinator to join our team on a casual basis (3 days per week, 8-hour shifts). In this role, you will provide direct nursing support to participants while ensuring their health, safety, and well-being. You will also play a key role in training and supporting staff to deliver high-quality, individualised care.

Key Responsibilities
  • Deliver person-centered nursing care to NDIS participants
  • Develop, implement, and review individualised care plans
  • Provide clinical guidance, supervision, and training to support workers
  • Conduct health assessments and ongoing monitoring
  • Support behaviour management strategies, including restrictive practices
  • Collaborate with families, carers, and multidisciplinary teams
  • Maintain accurate documentation and ensure NDIS compliance
  • Respond to complex care needs and crisis situations
Skills & Attributes
  • Strong client-focused approach with excellent communication skills
  • Ability to adapt communication style to suit different audiences
  • Sound computer literacy (Microsoft Office, email systems)
  • High level of confidentiality and professionalism
Key Competencies
  • Minimum 2 years 'experience in disability, mental health and complex care
  • Minimum 2 years' experience in community nursing (preferred)
  • Understanding of restrictive practices and behaviour support
  • Experience supporting individuals with challenging behaviours
  • Experience in Supported Independent Living (SIL)
  • Experience supporting people living with dementia
  • Experience in training and developing staff
Qualifications & Requirements
  • Current AHPRA registration as a Registered Nurse
  • Experience in disability, community nursing, or allied health settings
  • Ability to lead care teams and manage conflict
  • Strong skills in care planning, report writing, and compliance
  • Ability to work autonomously and within a multidisciplinary team
  • Excellent time management and organisational skills
  • Current Driver’s Licence
  • NDIS Worker Screening Clearance
  • Working with Children Check (Blue Card)
