Licensed Practical Nurse LPN - (BACT) at RiseWell Community Services – West Babylon, New York

RiseWell Community Services is seeking a dedicated Licensed Practical Nurse (LPN) to join our Assertive Community Treatment (ACT) Team in West Babylon, NY. This part-time position offers a unique opportunity to make a meaningful impact in the lives of individuals living with serious mental illness by providing community-based, recovery-oriented support.

JOB FUNCTION: Provide management and administration of medications, in conjunction with the psychiatrist, and registered nurse as a member of a mobile, multidisciplinary treatment team for persons with severe and persistent mental illness.

ESSENTIAL FUNCTIONS:

1. Responsible for conducting routine mental status exams.
2. Assesses physical health needs.
3. Makes appropriate referrals to community physicians and community based services.
4. Provides management and administration of medications and IM injections in conjunction with psychiatrist.
5. Participates on team that delivers an integrated array of treatment, rehabilitation and support services, customized to each person. Shares in the joint responsibility for integrated plan. Participates in team interactions producing regular ongoing transfer of information and skills.
6. Provides the majority of services in community. Participates in rotating 24-hour crisis coverage and response.
7. Comply with agency Compliance Plan and False Claims Policy by preventing, detecting and reporting any potential healthcare abuse and fraud. Report any potential non-compliance and comply with all other federal, state and local regulations applicable to this job description.
8. Utilize all agency based IT applications pertaining to this job description in a proficient manner.
9. Remain current on changing developments in individual field of expertise.
10. Perform all other job duties as assigned.

COMPETENCIES:

• Knowledge of nursing assessment methods.
• Ability to work within team setting.
• Skilled in counseling techniques through education, or training, or work experience.
• Ability to work as a generalist.
• Must maintain licensed as a practical nurse in New York State.

Hours: 21 hours/week

QUALIFICATIONS:
Currently licensed as a Licensed Practical Nurse by the New York State Education Department. Valid and clean NY State driver license. Minimum of 1 year of psychiatric experience in a treatment or rehabilitation setting. Fingerprinting, criminal record check, approval from NYS Office of Mental Health.
