About This Position
Job Summary
We are seeking compassionate, dependable, and dedicated Home Health Aides (HHAs) to provide high-quality in-home care to clients with diverse needs. The ideal candidate will assist with activities of daily living (ADLs), monitor patient health, and support clients in maintaining their independence and well-being under the direction of a Registered Nurse. This role offers an opportunity to make a meaningful difference in the lives of individuals requiring home and community care services, including seniors, individuals with disabilities, and those recovering from illness or injury. Responsibilities
- Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, and toileting
- Monitor and observe patient conditions, reporting any changes or concerns promptly
- Take vital signs such as blood pressure, pulse, temperature, and respiration rates
- Support patients with mobility using equipment like Hoyer lifts or transfer belts to ensure safety during transfers
- Provide medication reminders
- Provide companionship and emotional support to promote mental well-being
- Prepare meals according to dietary guidelines and assist with feeding if necessary
- Perform light housekeeping tasks such as laundry, cleaning, and maintaining a safe environment
- Document patient care activities accurately using EMR/EHR systems in compliance with HIPAA regulations
- Assist with specialized care needs such as catheter care, dementia or Alzheimer's care, hospice support, or memory care
- Support clients in the community setting by coordinating appointments
Communication & Team Collaboration
- Maintain clear and professional communication with clients, family, and team members
- Participate in care coordination by providing timely updates and feedback
- Represent the agency with professionalism and compassion in every interaction
Travel Requirements
- Travel to clients homes within the assigned service area
- Willingness to drive up to 45-60 minutes between shifts or visits as needed
- Manage schedule efficiently to ensure reliable and consistent client care
Skills
- Proven experience working with seniors or individuals with disabilities in home health or long-term care settings
- Knowledge of medical terminology and clinical procedures related to patient observation and care management
- Ability to assist with ADLs and use medical equipment safely and effectively
- Familiarity with EMR/EHR systems for accurate documentation of patient records
- CPR certification and knowledge of emergency response procedures
- Strong communication skills for interacting effectively with patients, families, and healthcare teams
- Compassionate caregiving attitude with patience and empathy for diverse client needs
- Ability to work independently while following care plans precisely
- Experience working with people with developmental disabilities or in hospice environments is a plus

Qualifications
- Current Home Health Aide (HHA) certification (or CNA if applicable per state requirements)
- Experience in home care, home health, or senior care settings preferred
- 6-12 months of caregiving experience preferred
- Compassionate, dependable, and service-oriented mindset
- Ability to follow care plans and work independently in a home setting
- Strong communication and interpersonal skills
- Valid driver's license, reliable transportation, and current auto insurance
- Willingness and ability to travel within the service area, including some extended drive times
- Current CPR certification preferred
