Minimum Qualifications:
- Registered Dietitian (RD)
- Bachelor’s degree in Nutrition/Dietetics
- Must have 2+ years of clinical experience in long-term care/acute care environment
- Excellent customer service skills
- Ability to collaborate between multiple disciplines/departments
- Strong communication/documentation skills, experience with PCC
- Experience working with elders living with dementia
- Experience working with onsite dialysis unit

SUMMARY: The Culinary Cook is responsible for preparing high‑quality, nutritious meals for elders at A.G. Rhodes. This role ensures that all food is prepared according to dietary guidelines, elder care plans, and sanitation standards. The Cook plays a key part in creating a positive dining experience that supports elder health, comfort, and satisfaction. Works with fellow Culinary Services team members to ensure elder customer satisfaction at A.G. Rhodes.
ESSENTIAL DUTIES AND RESPONSIBILITIES: include the following. Other duties may be assigned as needed.
- Provide medical nutrition therapy (MNT) in accordance with physician referrals, evidence-based guidelines, and established standards of practice
- Conduct comprehensive nutrition assessments, including diet history, anthropometric data, biochemical data, and medical history
- Develop, implement, and monitor individualized nutrition care plans to address health conditions, nutrition risk, and wellness goals
- Provide nutrition education and counseling to clients, patients, and families to support behavior change and improved health outcomes
- Monitor patient progress and modify care plans based on response, outcomes, and changes in medical status
- Collaborate with physicians, nurses, and interdisciplinary team members to coordinate care
- Maintain accurate, timely, and compliant documentation of assessments, interventions, and outcomes
- Ensure compliance with applicable federal, state, and organizational regulations, policies, and professional standards
