About This Position
PAY RATE: $32.48 - $48.72
BASED ON RELEVANT EXPERIENCE
This position is responsible for: Providing RN Care Management services designed to improve individual and/or family/ significant other overall health and well-being to a specified/targeted patient group and their families. Using various assessments to identify actual needs and potential risks for adverse outcomes, the RN Care Manager coordinates comprehensive health care and community-based services, home visits, and assessments to meet the needs and risks identified. Develops, implements, coordinates and evaluates patient care plans in an outpatient environment; collaborates with other health care providers, community service agencies, faith-based agencies, as appropriate, to coordinate care and services. The Care Manager uses clinical expertise and problem solving skills to achieve optimal clinical and resource outcomes within effective time frames. The underlying objective is to enhance the quality of clinical outcomes and patient satisfaction while managing the cost of care.
This position requires full understanding and active participation in fulfilling the Mission of Blessing Corporate Services. ICARE standards must be demonstrated. Adherence to Code of Conduct and other policies is required.

Graduate of an RN School of Nursing

Excellent written and verbal communication skills

Previous care coordination experience.

Candidates should possess a positive attitude, have a high level of energy, work in a team environment, and be resourceful for an evolving program.

Adequate transportation is a must.

Possesses current State RN license.
