About This Position

Community Health Worker

The compensation range noted above represents the base salaries for all positions at a given grade across the health system. Typically, a new hire can expect a starting salary somewhere in the lower part of the range. Actual salaries are determined based on the candidate's relevant experience. No employee will be paid below the minimum of the range.

Holyoke Medical Center is an independent community hospital that has served the healthcare needs of the Pioneer Valley for over 125 years. We continue to grow and address the needs of our community through quality and patient-centered care. The CHW provides chronic diseasea education, smoking cessation counseling and links patients to mental health resources and other social determinants of health.

We are seeking a Community Health Worker who, under the general direction of the Senior Community Navigator and Manager, collaboratively provides comprehensive care management services in response to e-referrals, and assists patients/clients to navigate and access community services that will have a positive impact on their overall health. This position requires a non-smoker for at least 2 years .

REQUIREMENTS: High School Diploma or equivalent; bi-lingual or multi-lingual preferred; driver's license and vehicle; must complete smoking counselor training within one year of hire; completion of CHW Core Competency Education in Massachusetts; trained and certified in chronic disease management such as asthma, hypertension, diabetes and mental health; pediatric experience a plus
