Overview

JOB DESCRIPTION:

The Nurse Navigator provides procedural coordination to the Cardiology Program to include, but not limited to: Obtaining Insurance Pre-Authorization, coordinating Watchman, TAVR, and Mitra Clip. The nurse works collaboratively with the Cardiovascular Administrative Director, Regional Director of Cardiology Operations, physicians from multi-specialties in developing referrals, coordinating clinic and outpatient visits to navigate clinical care accurately and effectively. The Program Navigator serves as primary coordinator for Watchman LAAO program. Coordinating comprehensive care for patients from initial referral through two years post procedure. The Program Navigator provides clinical education and support to other nursing departments in respect to procedures and testing. The Nurse Navigator monitors and assures that patient care is in compliance with CMS required clinical registries associated with any high-tech procedures.

Additional responsibilities:

Conduct telephone clinical assessments, follow up and care planning to improve outcomes and reduce readmissions.

Perform comprehensive chart reviews and ensure guideline-based eligibility.

Facilitate medication reconciliation and address barriers to care, such as pharmacy access, follow up appointments and transportation challenges to reduce readmission risk.

Track program outcomes, quality metrics and registry data for reporting.

Navigate insurance authorizations, appeals and benefit coordination

Ensure compliance with CMS and institutional clinical protocols

QUALIFICATIONS:

  • Skilled in cardiology, electrophysiology or catheterization lab experience.
  • Basic cardiovascular rhythm recognition and cardiopulmonary monitoring assessment.
  • Advanced knowledge of cardiovascular diagnostics and imaging techniques.
  • Excellent interpersonal and communication skills, written and verbal.
  • Ability to problem solve and multitask.
  • Highly organized in compiling clinical data / images for presentation to multi-specialty physician teams to accurately assess patients for advanced clinical procedures.
  • Diplomatic personality with strong drive to grow the program.
  • Registered Nurse with a minimum 3 years cardiology, electrophysiology or cath lab experience.
